On 11/21/2023 Licensing Program Analysts (LPA) Judy Laureano conducted an unannounced Annual Required Inspection at the above mentioned facility. LPA was greeted by Violet Ferguson Edwards, facility director, who guided LPA on a tour. Facility is a combo center with an infant license on the same site; infant license number: 191602932.

Facility operates Monday through Friday 6:30 a.m. to 6:00 p.m. Facility is licensed to serve children ages 2 years through 5 years of age for a max capacity of 78 children.


The following was observed:
Bear Classroom: 6 children and 1 staff member with director
Lady Bug Classroom: 3 children and 1 staff member
Star Classroom: 10 children and 2 staff members
Rainbow Classroom: no children were observed in the classroom.

Classrooms was observed and inspected. Classrooms were observed with age-appropriate materials and a variety of manipulatives. LPA observed the First Aid kits and emergency bag packs in the classroom. Facility is open to enroll children that might need medication administered. Medication was observed stored in the top cabinet of the classroom of whom medication belongs too. LPA reminded director to ensure medication is its original content with an unaltered label with current prescription and all necessary LIC forms. Allergy list was observed on the refrigerator door in the classroom.

There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. LPAs observed a centralized Fire Alarm throughout the facility.

Facility has a designated cleaning crew that cleans and disinfects the facility after hours. Teachers are responsible for the day-to-day cleaning in the classrooms. Classrooms were observed to have cleaning solutions in the top cabinet by the sink area; safety latch was observed making the content in accessible to the children in care. Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. Areas were observed to have soft cushioning materials and a variety of shaded areas for children to play.

All toilets and hand washing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. Solid waste storage containers have tight-fitting covers and are in good repair. LPA requested facility director to replace and/or add a lid to the trash bin in the Bear and lady bug classroom.

Facility provides snacks and menus was observed in each classroom’s refrigerator door. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. Copies of the menus as served shall be dated and kept on file for at least 30 days. Drinking water was observed via filter water in the classroom. Children bring in their refillable water bottle and teachers are responsible for refilling as needed. Parent board was observed with all necessary posting, parent board in the classrooms. LPA provided director with updated Parent’s Rights Poster.

The facility is free of flies, insects and rodents. Facility contracts with a Pest control vendor to ensure that facility stays pest free. LPA observed working fire extinguisher and carbon monoxide detector throughout the facility. All electrical outlets were observed to be covered.

Prior to working or volunteering in a licensed childcare facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption.
Capacity and limitations as specified on the license are being maintained.

At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. Current CPR and Pediatric First Aid was reviewed in teacher’s file. LPA discussed the importance of making sure all Pediatric CPR and Pediatric First Aid training is EMSA approved. Mandated Reporter training was reviewed. On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training.

LPA reviewed a sample of children’s files and observed 5 files completed. LPAs reminded facility director to ensure all children’s file have all the necessary required forms and provided and updated LIC 311A and LIC 125 to use as a reference when auditing files.

The name of the child care center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. LPA observed hard copies of sign in and out sheet in the classroom. All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 12 children in care.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.

Facility director, Violet Ferguson Edwards was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

A notice of site visit was given and must remain posted for 30 days.
