On 1/11/2023 Licensing Program Analyst (LPA) Judy Laureano conducted an unannounced Annual Required Inspection at Southern California Montessori School located at 1430 Centinela Avenue, Los Angeles, CA 90025 and met with Director Ann Abellara. LPA toured the facility indoors and outdoors and a census was taken. LPA observed 18 children with 4 staff members providing care and supervision.

You are able to access the facility through the main entrance located by the facility’s parking lot. This is a full day program and operates from 7:30 a.m .to 6:00 p.m. Monday through Friday.

Facility is located in the ground floor of a church. Classroom space was observed and inspected. Space is divided in two classrooms. The space across the kitchen, LPA observed a small leak that has been made inaccessible to the children. Director confirmed that due to the heavy rains, facility experience a small leak and a facility has already contracted for repairs. The area is barricaded with a children’s size shelve that makes the area inaccessible.

There is no swimming pool or other bodies of water on the premises. No Fire arms or weapons were observed on the premises. Medications are made inaccessible and stored in the Director’s office which is located on the second floor above the classroom. If medication needs to be refrigerated, medication will be stored in the kitchen area. Kitchen area is only used to warm up children’s snacks and lunches if needed. All necessary paperwork that needs to be included with medication is stored in the director’s office.

Teachers do the day to day cleaning in the classroom. Disinfectants, cleaning solutions and other hazardous items are made inaccessible and stored in a locked cabinet outside the kitchen and front entrance closet.

Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazard

All toilets and handwashing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe.

Parents provide lunch and snacks in adequate containers such as thermos or bags with ice packs. All food is protected against contamination and any contaminated food is discarded immediately. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin.

Solid waste storage containers have tight-fitting covers and are in good repair. Staff is responsible for day to day cleaning and disinfecting. Pest control is contracted by the church to ensure facility stays pest free.

Drinking water is available both indoors and outdoors. Each child brings in their own refillable water bottle and teachers are responsible for refilling. LPA observed filter water in the kitchen.

Areas around high climbing equipment, swings and slides have cushioning material to absorb falls. The facility is free of flies, insects and rodents. Facility a centralize fire alarm and sprinkler system. LPA observed working fire extinguisher in facility.

Prior to working or volunteering in a licensed childcare facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption. Capacity and limitations as specified on the license are being maintained.

At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. Director’s CPR and Pediatric First Aid was completed on 1/22/2022. LPA reviewed Mandated Reporter Training and staff has current proof of completion. LPA reviewed staff files and 3 out of 1 files need Mandated Reporter Training to be completed. LPA issued a Technical Advisory and director agrees to review files and ensure all necessary documents are completed. LPA reviewed a sample of children’s files and observed all files were complete. LPA provided director with a copy of the LIC 311A to use as a reference to ensure all files have current documents.

The name of the child care center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 12 children in care.

Facility representative, Ann Abellera, was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violation.

Incidental Medical Services (IMS)currently being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. LPA and Director discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Exit interview conducted and report was reviewed with the licensee facility representative Ann Abellera.
