On 10/19/2022 Licensing Program Analyst (LPA) Judy Laureano conducted an unannounced Annual Required Inspection at Temple Isaiah located at 10345 W. Pico Blvd., Los Angeles, CA 90064 and met with Director Tamar Andrews and Avital Etehad Associate Director. LPA toured the facility indoors and outdoors. LPA inspected all licensed classrooms.

LPA observed the following:
Yellow Submarine- 15 children and 3 staff members
Beautiful Butterfly- 11 children and 3 staff members
Red Caboose- 17 children and 2 staff members
Hungry Caterpillar- 17 children and 2 staff members
Minnows- no children present and 1 staff member
Pollywogs- 11 children and 2 staff members
Guppies- no children- ½ day preschool classroom
Racing Rockets (Room 302A)- 15 children and 3 staff members
Glittery Galaxy (Room 302B)- 17 children and 4 staff members- Children were in the playground
Silver Moon (Room 301)- 15 children and 2 staff members

Classrooms currently not in use: Room #201, Room #203, Room 212, Room 210/211, Room 300B

Currently Facility Operates Monday through Friday from 8:00 a.m. to 5:00 p.m. Facility currently has a play yard waiver that needs to be posted next to facility's license along with the play yard schedule.

There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions and other hazardous items are made inaccessible in each classroom. Teachers are responsible for day to day upkeep of the classroom. Facility has a cleaning crew that comes in to assist with cleaning. Medication is kept in the Director’s office, locked and made inaccessible to the children in care.

No poisons were observed during the inspection. Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards.

All toilets and handwashing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. All food is protected against contamination and any contaminated food is discarded immediately.

Solid waste storage containers have tight-fitting covers and are in good repair. Facility provides snacks and families are responsible for providing meals in adequate packing, eg. Ice packs in lunch bags and/or thermos. Drinking water is available both indoors and outdoors. Children bring in their own water bottle and meals from home.

Areas around high climbing equipment, swings and slides have cushioning material to absorb falls. The facility is free of flies, insects and rodents. Facility has a centralize fire alarm and smoke detector system that includes a sprinkler system. All classrooms were observed to have a working carbon monoxide detector.
Facility uses an online platform, Playground, to communicate with parents regarding enrollment forms, facility updates and any other information that needs to be disseminated.

Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption.

Capacity and limitations as specified on the license are being maintained. Facility has a capacity of 306.

At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. LPA reviewed 6 staff’s file and all files had current CPR and Pediatric First Aid and Mandated Reporter Training. LPA reviewed a sample of children’s files, (5), and observed all files were complete. LPA will provide facility admin with a copy of LIC311A and LIC 125 to use as a guide to audit files and program.

The name of the child care center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. All children are under supervision, including visual supervision, of a teacher at all times.

Facility maintains a ratio of one teacher supervising no more than 12 children in care.

Facility representative, was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) are currently being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. Information regarding Americans with Disability Act (ADA) can be obtained by contacting US Department of Justice toll free ADA Information line at (800) 514-0301(voice), (800) 514-0383 (TDD) and website link https://www.ada.gov/childqanda.htm.

LPA and Director discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
