On 4/16/2024 Licensing Program Analyst (LPA) Judy Laureano conducted an unannounced complaint investigation to deliver findings of above mentioned allegations. LPA met with Avital Etehad, facility director. LPA toured the facility indoors and outdoors and observed 115 children and 31 staff providing care and supervision.

The investigation of the above mentioned allegations was conducted by California Department of Social Services CDSS Investigator Spindola and LPA Laureano and Castellanos.

On 2/08/2024, Licensing Program Analysts (LPA) Judy Laureano and Cristina Castellanos conducted an unannounced complaint investigation regarding the above allegations. LPAs arrived at facility at approximately 9:00 a.m. and met with Avital Etehad, facility director. LPAs toured the facility indoor and outdoors and observed 125 children and 34 staff members.

LPA requested and reviewed copies of children’s roster, parent handbook/and or contract, staff roster and copies of incident reports. LPAs initiated interviews with director, staff and parents.

On 2/22/2024, LPA Laureano and LPA Castellanos conducted a subsequent investigation at facility. LPAs observed 127 children and 27 staff members present during the inspection. LPAs interviewed children and staff.

On 3/11/2024 California Department of Social Services CDSS Investigator Spindola completed investigative interviews with relevant parties. On 4/16/2024 LPA Laureano completed all investigative interviews.

Based on the investigation conducted by CDSS Investigator Spindola, interviews, LPA’s review of documents, interviews with director, staff, children and parent, there is not enough information to prove or disprove facility violated the children’s personal rights. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove that the above allegations did or did not occur. Therefore, the allegations are found to be unsubstantiated.

Exit interview was conducted with Facility Director, Avital Etehad and a copy of the report and Notice of Site Visit was provided.
