On 01/31/2024, Licensing Program Analyst (LPA) Cristina Castellanos and LPA Ranita Richmond conducted an unannounced Annual Required Inspection at the above-mentioned address. LPAs Castellanos and Richmond met with Director Dorian Theberg. LPAs toured the facility indoors and outdoors and observed 36 children in care, being supervised and cared for appropriately by 6 fingerprint cleared staff members. Facility hours of operation are 7:30am – 5:00pm.

Capacity and limitations as specified on the license are being maintained.

All children are under supervision, including visual supervision, of a teacher at all times. The CCC is in compliance with Title 22 regulations regarding Staff to Student ratios. Parents sign their child in/out of the facility. LPA Castellanos and Richmond advised Director to ensure that parents sign using their full legal name and the time of day.

There are no swimming pools, ponds, or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items were accessible to the children in care. All poisons were not kept in a locked cabinet area. In the Cubs Classroom LPAs observed two Lysol cans and Clorox wipes in the staff members’ restroom which was not locked. Additionally, LPAs observed 5 bottles of cleaning solutions underneath the unlock sink cabinet in the Pre-K Lions Classroom, which can potentially pose health, safety, or personal rights risk to children in care. LPAs observed Master Teacher Martinez remove all bottles underneath the sink and then threw them away in the outdoor trashcan. LPAs observed Director Dorian removed all cleaning solutions from the staff member restroom and placed them on a top shelf in front of the restrooms.

LPAs Castellanos and Richmond observed three (3) fully charged fire extinguishers throughout the facility. LPAs observed 2 working carbon monoxide detectors. In addition, the smoke detectors are wired to a Pull Fire Alarm system throughout the facility.

All toilets and hand washing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. LPAs Castellanos and Richmond inspected the classrooms and observed age-appropriate toys, books and furnishings. Furniture and equipment are in good condition, free of sharp, loose, or pointed parts. Playground equipment is in safe condition, free of sharp, loose, or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. Areas around high climbing equipment, swings and slides have cushioning material to absorb falls.

LPAs reminded Director that any areas designated as OFF-LIMITS should be made inaccessible during the hours of operation and/or while children are present. Facility shall ensure the inaccessibility to areas (Elementary Grounds) not licensed by CDSS.

LPA Castellanos and Richmond conducted a Health and Safety inspection of the kitchenette, and all storage areas are clean, free of litter/rubbish and free of rodents/vermin. Solid waste storage containers have tight-fitting covers and are in good repair. Director confirmed that the facility does not provide meals or snacks, only filtered water. Per Director parents assure they provide the meals and snacks for their children, which comply with dietary restrictions and allergies. LPA discussed the importance of maintaining a system where allergies and food restrictions are noted.

LPAs Castellanos and Richmond reviewed 10 children’s files and observed files to be in compliance including current contact information for authorized representatives and/or relatives who can assume responsibility for the child, immunization records, and authorization for medical treatment.

LPAs Castellanos and Richmond reviewed 7 personnel files and observed files to be in compliance with current CPR/First Aid and Mandated Reporter Training. Immunizations Records for personnel in childcare are missing 7 out of 7 Flu(Influenza) vaccine/declinations, which can potentially pose health, safety, or personal rights risk to children in care.

CCC COMPLETED TESTING AND HAS REMEDIATING LEAD EXCEEDANCES. LPA Castellanos and Richmond verified that the lead testing was completed in accordance with Written Directives outlined in PIN 21-21.1-CCP.



Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center.

A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

This facility provides Incidental Medical Services - IMS. Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

LPAs Castellanos and Richmond reminded Director to access the Community Care Licensing website www.ccld.ca.gov for access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, there were two (2) deficiencies cited at this time for violation of Title 22 regulation; (see LIC809-D).



An exit interview was conducted and Plans of Correction were reviewed and developed with Director Dorian. A copy of this report and appeal rights were discussed and left with Director Dorian. A Notice of Site Visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
