On 6/27/2019, Licensing Program Analysts (LPA) Helen Estrella arrived at the facility to conduct an annual random inspection at the facility. LPA met with Tarisha Jayasuriya, Center Director who guided LPA on a tour of the facility (inside and outside).

The facility consists of an infant center with a combination of a preschool located in the front of the facility. LPA observed total of 6 infants supervised by 2 teachers in classroom #1 and 5 toddlers supervised by 2 teachers in classroom #2. LPA reviewed sign in/out sheet and verified that all staff has obtained criminal record clearances and were associated to the facility.

There were no baby walkers on the premises. Sanitary solution (bleach solution) is out of reach to infants in care. Changing tables have at least a 1-inch-thick padding covered with washable vinyl or plastic. Changing table is within an arm's reach of a sink. There is 2 toilets and 1 sink and there is sufficient toilet paper and paper towels in the restroom.

Furniture and equipment appear age appropriate and in good repair. Toys are found to be safe with no sharp edges, splinter or points. There are no small parts that can be pulled off and swallowed. Cribs and playpens are available for all infants and cots for children who are able to climb out of a playpen and/or crib. Bedding is separately identified and stored for each infant/toddler. Placement of cribs and mats allow for entry to/exit from the napping space. Director states the isolation area for ill children is in her office. Menu of snacks was observed. The kitchen was inspected and food is labeled for each child appropriately. Smoke/Carbon monoxide detector inspected. First Aid equipped in the kitchen and staff observed with Pediatric First Aid CPR valid through 05/2020.

Outdoor area was inspected and it was observed there are no sharp items that pose a hazard to children in care. There is drinking water readily available. The facility not provides Incidental Medical Services (IMS). LPA reviewed medication properly labeled and stored and observed allergy list available for staff in kitchen. Children and staff records were reviewed and it was observed to be complete with Community Care Licensing Records requirements.

Facility informed children records and facility roster must be kept for 3 years and advised all public reports must be kept for review. Forms and Regulations available at: www.ccld.ca.gov. Facility was informed of appeal rights, must be in writing within 15 business days from date of receiving penalty assessment.

The facility was informed of the responsibility to report suspected Child Abuse by calling the Child Abuse Hot line at 1-800-540-4000. Also call the CCL office within 24 hours of the Unusual Incident and follow up with a written Unusual Incident/Injury Report (LIC 624B) within 7 business days.

The facility was reminded that all infants must be placed on their backs when sleeping to prevent S.I.D.S. (Sudden Infant Death Syndrome), and that the Provider is required to wash hands after every diaper change and to never shake a baby to prevent the Shaken Baby Syndrome. Facility was reminded that only children eating may be in high chairs and that car seats are utilized only for transportation.

Facility was recommended the following safe sleep practices: always place infants on their backs for sleeping; use only a tight-fitting sheet on the crib or play yard mattress; do not hang any items from the crib or above the crib; keep all items, including blankets, out of the crib or play yard; pacifiers may be used as long as they do not have items attached to them; infants should not be swaddled or have any items covering them while sleeping; the temperature of the room should be comfortable enough for an adult to wear a t-shirt and not be too hot or too cold. Please note, these guidelines are recommendations for best practices only, until regulations are approved and adopted.

The licensee was advised that, once licensed, the Notice of Site Visit must be posted at the entrance of the facility for a period of 30 days. If a serious violation is cited, (Type A violation), a copy of the licensing report (LIC809 or LIC9099) must also be posted for 30 days. If these requirements are not met, civil penalties in the amount of $100 per violation will be assessed. The applicant was made aware that a licensee may file an appeal, in writing 15 business days from the date of receiving the penalty assessment.

The facility appeared to operate within substantial compliance. A copy of this report was provided to the Director and exit interview conducted.

This facility provides Incidental Medical Services – (IMS). LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Incidental Medical Services Include: Blood-Glucose Monitoring for Diabetic Children, Administering Inhaled Medication, Administering EpiPen Jr. and EpiPen or other Epinephrine Auto-Injectors, Glucagon Administration, Gastrostomy Tube Care (G-tube care), Insulin Injections Administration, Anti-Seizure Administration, and Emptying an Ileostomy Bag.

CCAP participates in many community activities and special projects in order to disseminate information on the State’s licensing role, provide information to the public and parents on child care licensing, and provide many other helpful resources to the licensees and the public. New Immunization Requirement: Law enacted by SB 277, beginning January 1, 2016, personal beliefs exemptions will no longer be an option for the vaccines that are currently required for entry into child care or school in California. Personal beliefs exemptions already on file will remain valid until the child reaches the next immunization checkpoint.

Mandated Reporter: Beginning on January 1, 2018, AB 1207, requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years.
