Licensing Program Analyst (LPA) Rita Ramos conducted an unannounced required 1 year inspection to the above facility on 11/04/2021. LPA arrived at the facility at 11:50 AM, identified self and met with Nancy Perez, Coordinating Teacher, who guided analyst on a tour of the facility. LPA provided Coordinating Teacher with a copy of the LIC 125 Entrance Checklist to help facilitate the inspection. This is a preschool program which consists of 2 classrooms; RM 1 and RM 2 and they each operate 2 sessions. Facility operation hours are Monday to Friday from 8AM to 11AM for the AM sessions and 12PM to 3PM for the PM sessions. LPA was later met by Robbie Waddell, Coordinating Teacher. This facility is located within the premises of Abraham Lincoln Elementary School in the city of Long Beach.

All areas identified on the Facility Sketch were inspected. During the inspection children for the PM session arrived and LPA observed the following staff were in the classrooms: Room 1: Staff #1, #2 and #3 with 15 preschoolers; Room 2: there were no staff or children present due to current enrollment. The facility was observed to be within the license capacity and limitations. The following was observed during the tour of the facility:

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Children at this facility do not nap due to part-day enrollment. Per Coordinating Teacher, Nancy Perez, the isolation area is located in the nurse's office. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Availability of indoor drinking water was observed in classrooms.



Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Per Coordinating Teacher Nancy Perez, there are no poisons stored at the facility. When inspecting the facility for carbon monoxide detectors, --------------------Page 1 of 3

LPA observed that the facility had 2 (one in each classroom), however, neither of the carbon monoxide detectors were operable. LPA and Coordinating Teacher Nancy Perez opened the carbon monoxide detectors and observed that neither had batteries. Batteries were placed during the inspection. Both LPA and Coordinating Teacher Nancy Perez tested the carbon monoxide detector at approximately 12:05PM to ensure that they were operable. LPA issued an Advisory Note-Technical Violation during the inspection for the carbon monoxide detectors.

This facility has all meals and snacks delivered to the classrooms from the elementary school's cafeteria. LPA inspected the cafeteria/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Outdoor drinking water is made available via a water pitcher and disposable cups. LPA advised that no children shall be left without the supervision of a teacher at any time.

All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic-free There are no firearms stored on the premises. There are no pools or bodies of water at the facility.

All staff and employees have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Long Beach Unified School District.

There is at least one person trained in CPR and Pediatric First Aid present during this inspection. Children’s Records were reviewed and documented on the LIC 857.

LPA also reviewed staff records. The review of Staff records was documented on the LIC 859. In review of staff records, LPA observed that Staff #4 did not have the required AB 1207 Mandated Reporter Training certificate on file. Per Coordinating Teacher Robbie Waddell, Staff #4 does not have proof of a current AB 1207 certificate on file. Coordinating Teacher Nancy Perez informed LPA that Staff #4 -----Page 2 of 4

took an AB 1432 Mandated Reporter Training. LPA informed Coordinating Teacher, Nancy Perez, that Staff #4 is required to take the AB 1207 training. LPA issued an Advisory Note-Technical Violation during the inspection. Coordinating Teacher Nancy Perez had Staff #4 register and sign-in to take the AB 1207 training during the inspection to ensure compliance. All staff have been given on the-job training sanitation principles, housekeeping, including universal health precautions.

During the inspection LPA was informed that the new appointed designee to oversee the day to day operations at the facility is Coordinating Teacher, Nancy Perez as of 11/01/21. The previous designee is Coordinating Teacher, Robbie Waddell which is why LPA was met by both during the inspection. LPA advised that posted forms such as the Emergency Disaster Plan and LIC 308 Designation of Facility Responsibility be updated to reflect the change.

Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed. Children present were signed in. Disaster drill log was available, last drill was conducted on 10/23/21.

Menus were observed to be posted. Preschoolers in the AM session are provided with a full breakfast and children from the PM session are provided with a full lunch.

First Aid supplies were observed in the classroom. Per Coordinating Teacher Nancy Perez, there are currently no children on medication.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
