Licensing Program Analysts (LPAs) Rita Ramos and Fabiola Vasquez conducted an announced case management inspection due to Licensee requesting to relocate the licensed classrooms. LPAs met with Cristabel Rodriguez, Program Coordinator, who guided analysts on a tour of the facility. Also present during the inspection was Nancy Perez Villegas, Coordinating Teacher. This facility is located at McKinley Elementary School in the city of Long Beach.

This facility was previously inspected for a temporary room relocation on 01/04/19 due to the elementary school being under construction. The facility was originally licensed for Rooms 4 and 7, then temporarily relocated to Rooms 34 and 35. Since construction is over, the facility is returning to the original licensed classrooms, however, Room 4 is now titled Room 1102 and Room 7 is now tilted Room 1105. LPAs measured Rooms 1102 and 1105 during the inspection to ensure that the capacity originally licensed for will still be reflected.

This program will serve children ages 2 years to 5 years. This program will operate Monday through Friday. Room 1102 and Room 1105 will operate two sessions. The AM session will operate from 8:00AM to 11:00AM and the PM session will operate from 11:45AM to 2:45PM.

All areas identified on the facility sketch were toured both indoors and outdoors.

This facility will have parents and/or guardians sign children in and out electronically via a computer tablet. The tablets will be located in each classroom. LPAs advised the Program Coordinator and the Coordinating Teacher that Sign-In and Sign-Out sheets should be readily available to print if Licensing were to request them upon an inspection. A health check will be conducted prior to children signing in on a daily basis. There is drinking water readily available in each class via a water faucet.

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Children will not nap at this facility. Per Coordinating Teacher, the isolation area is located in the nurse's office. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. LPAs observed that facility does have a carbon monoxide detector, smoke detector, and a serviced fire extinguisher.

Disinfectants, cleaning solutions, and other items that are dangerous to children, will be inaccessible to children and stored in lock cabinets in each classroom.

This program will provide breakfast to the children in the AM session and children from the PM session will be provided with a lunch. A monthly snack menu is available and posted.

Prior to inspecting the cafeteria, Licensee had requested to continue the use of a cafeteria waiver in place, however, LPA Ramos advised the Program Coordinator that a cafeteria waiver was granted for a capacity of 24. The Program Coordinator provided LPAs with an updated cafeteria waiver in case the Department were to require it. LPAs observed that the kitchen areas/food preparation areas and food storage areas were observed to be clean. LPAs did however, observe 2 dead roaches on the floor of the cafeteria where the eating benches are located. LPAs further observed that the drinking water faucet was not working and it was broken. Although no children were present due to the summer break, LPAs advised that the faucet be fixed and the roaches removed. The Coordinating Teacher removed the dead roaches during the inspection and the Program Coordinator placed a ticket for the faucet to be fixed.

Prior to inspecting the outdoor playground, Licensee had requested that the facility continue with the outdoor waiver that was originally in place prior to the relocation. LPA Ramos advised the Program Coordinator that there is no outdoor waiver on file to reflect the original license. LPA Ramos advised that a new waiver be provided to reflect the outdoor waiver for the capacity of 48. LPAs inspected the the outdoor yard to ensure that the equipment is in a safe condition, free of sharp, loose or pointed parts. LPAs advised the Program Coordinator and the Coordinating Teacher that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. LPAs advised the Coordinating Teacher to cover the edges of 3 water spouts on the yard. There is adequate shade in the play yard. Availability of outdoor drinking water was observed.-----Page 2 of 3

There are no bodies of water on the premises.

Coordinating Teacher stated that medication will be stored in a locked cabinet. First aid kits are available in each classroom and were observed to contain the required first-aid supplies.

The nurse's office room will be used as an ill/isolation area. Ill children will use the restroom located in the nurse's office if they were to be ill. There is a bed available for children to rest if needed. This room was observed to be conducive for children's use at this time.

Incidental Medical Services (IMS): This facility plans to provide Incidental Medical Services – IMS. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department. LPAs advised that forms, regulations and quarterly updates can be accessed on the Child Care Licensing website at: www.ccld.ca.gov.

Based on measurements taken today:

Room 1102 : space available for 24 preschool children.

Room 1105: space available for 24 preschool children.

Outdoor yard: space available 86 preschool children

A relocation license may be granted after a final application review. A fire clearance has been granted for 48.

The Notice of Site Visit (LIC 9213) – must remain posted for
30 days during the hours of operation after each site visit by a licensing representative. Exit interview was conducted with Cristabel Rodriguez, Program Coordinator, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role. -----Page 3 of 3
