On 9/8/2023 at 01:30 PM an unannounced Required – 1 Year Inspection was conducted by Licensing Program Analysts (LPAs) Katrina Chicote and Raul Navarro. Facility is currently licensed for a capacity of 48. Licensing staff announced purpose of visit and initially met with Randy Sanchez, who granted entry to facility. Facility is located on Garfield Elementary School Campus in bungalows 25101 and 25102. The program currently operates a AM (8-11am) and PM session (12-3pm). LPAs were taken on a guided tour of the facility of both indoors and outdoors. LPAs observed required licensing postings on the wall near the entrance. At the initial start of the site visit, there is a total census of 15 children and a total census of four staff present. The facility is made up of two classrooms. Currently the facility is only using one classroom.

Beginning at 1:30 PM, furniture and equipment were inspected for age appropriateness and good repair. LPAs observed surface of classrooms to be safe and clean, and child size tables and chairs available for children well maintained. LPAs observed learning stations for dramatic play, math, and science with toys well maintained free of loose and sharp parts and a library area available for quiet play. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings. Due to part day programs, children do not nap at the facility. Availability of drinking water was reviewed. LPAs observed water bottles available for children. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. LPAs observed bathroom available adjacent to the classroom. LPAs observed that the girls bathroom had two toilets and two sinks and the boys bathroom had one and a half toilets and two sinks. LPAs observed a third sink available inside the classroom.

Lunch menus were reviewed and observed posted at time of inspection. All meals are provided by the Long Beach Unified School District. Children are provided with a full lunch. Per CT, staff picks up lunch from cafeteria. Trash cans used to discard food have tight fitting lids and solid waste bags shall be discarded immediately after each meal.

A first aid kit is kept in the classroom. Carbon monoxide detector was tested and observed operable. LPAs observed smoke detectors observed present in the facility and fire extinguishers have been serviced. LPA observed disaster drill log available, last drill conducted in June 2023. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Coordinating Teacher states that there are no poisons stored at the facility. LPAs observed medicine that is stored in classroom to have required documents and to be stored per Title 22 Regulation requirements. CT states that sick/isolation area is in the nurses office and in the classroom that is not currently being used.

Outdoor equipment was inspected for safety, cushioning material, good repair and age appropriateness. LPAs observed shaded area and play structure to the right of outdoor play area that has cushioning material to absorb fall that is well maintained. LPAs observed bicycles, plastic playhouse, and other toys available for children to use outside that is well maintained. Required shade and fencing were inspected. LPAs advised CT that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to their water bottles during outside play. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. Personal Rights of children were observed by LPA.

Staff and children’s records were reviewed. Children’s Records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857. The review of Staff records was documented on the LIC 859.

Staff present did have proof of the AB 1207 Mandated Reporter Training certificate on file. All staff have been given on the-job training sanitation principles, housekeeping, including universal health precautions. LPAs observed Pediatric First Aid/CPR for staff is current. LPAs did not review confidential staff files during the inspection due to the staff confidential files being stored at the administrative office. All staff and employees have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Long Beach Unified School District.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES:
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

LPA referred facility representative to the Department website for lead: Lead Toxicity Prevention and Water Testing Information.

Facility representative was reminded that all adults 18 and over in the facility, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed facility.

The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked and batteries replaced as needed.

Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. LPA advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation.
