On 01/27/2023 at 11:30 AM an unannounced Required – 1 Year Inspection was conducted by Licensing Program Analyst (LPA) Katrina Chicote. Licensing staff announced purpose of visit and initially met with Staff 1(S1), Head Teacher, who granted entry to facility. Facility is currently licensed for a capacity of 45 and have 45 enrolled. Facility is located on Signal Hill Elementary Campus. The program currently operates three programs, two part day programs: AM Program 7:45 AM to 11:15 AM and PM Program 12:15 PM to 3:45 PM, and a Full Day Program from 7:45 AM to 2:45 PM. LPA was taken on a guided tour of the facility of both indoors and outdoors. LPA observed required licensing postings in each classroom. At a later time, Shannon Houston, Early Learning Center Manager (ELCM), and Jenny Acosta, Program Administrator, joined facility inspection.

At the initial start of the site visit, there is a total census of 21 children and a total census of five teachers present. The facility is made up of three classrooms: RM 1 - EHS Full Day, RM 2 - Part Day AM/PM Collaborative Co-Teaching (CCT) Program, and RM 3- Preschool Full Day Program . LPA observed eight children in RM 1 with two staff (S2 and S3), no children in RM 2, and 13 children with two staff (S4 and S5) in RM 3 at time of inspection.

Beginning at 11:36 AM, furniture and equipment were inspected for age appropriateness and good repair. LPA observed child size tables and chairs available for children well maintained. LPA arrived while children were eating lunch, and observed children eating lunch in the classrooms with a trash can that has tight fitting lid available near by. LPA observed learning stations for dramatic play, math, and science with toys well maintained free of loose and sharp parts and a library area available for quiet play in each classroom. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings. Cot were inspected and observed laid out in RM 1 and RM 3, cots were laid out that it does not block access or hinder walkways at time of inspection. Per S1, linens are provided by the facility and washed by a laundry service weekly. Availability of drinking water was reviewed. LPA observed water coolers available for children and cups. Age appropriate sinks and toilets were inspected for availability,

good repair, water temperature, toilet paper, area safety and sanitation. LPA observed bathroom available in RM 2 that is shared by program that has two toilets and one sink, with extra sinks available for use in each classroom. LPA observed diaper changing table in RM 1 that is on wheels and can be moved within arm's reach of sink when in use.

Snack/lunch menus were reviewed and observed posted at time of inspection. All meals and snacks are provided by the Long Beach Unified School District. Per Facility Representative, Full Day programs are provided breakfast, lunch, and snack; AM Program provided breakfast and PM Program provided lunch which is dropped off from school cafeteria. Trash cans used to discard food have tight fitting lids.



A first aid kit is kept in each classroom and was inventoried for supplies. At 12:12 PM, a carbon monoxide detector was tested and observed operable in RM 3. LPA observed smoke detectors observed present in the facility and fire extinguishers have been serviced on 09/01/2022. LPA observed disaster drill log available, last drill conducted on 01/12/2023. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. S1 states that there are no poisons stored at the facility and no children currently enrolled require medications. Per ELCM, isolation for sick children is in school's Nurse's Office.

Beginning at 11:47 AM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPA observed age appropriate multi-level play structure with slides that has cushioning material to absorb fall that is all well maintained. LPA observed bicycles, plastic play treehouse, plastic crawl tubes and other toys available for children to use outside that is well maintained. Required shade and fencing were inspected. LPA observed benches with shading available for children. LPA advised that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to a water cooler with paper cups during outside play. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed at time of inspection.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. LPA observed all children present signed in at time of inspection and observed AM program sign in/out sheet to have all signatures signed in and out. Personal Rights of children were observed by LPA.

Beginning at 12:25 PM, staff and children’s records were reviewed. Children’s Records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857. The review of Staff records was documented on the LIC 859.

Staff present did have proof of the AB 1207 Mandated Reporter Training certificate on file. All staff have been given on the-job training sanitation principles, housekeeping, including universal health precautions. LPA observed Pediatric First Aid/CPR expires on 12/2025 for S3. LPA did not review confidential staff files during the inspection due to the staff confidential files being stored at the administrative office. All staff and employees have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Long Beach Unified School District.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The following were discussed:
The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked and batteries replaced as needed.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today, 01/27/2023.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
