On November 9, 2023, at 1:41 PM, Licensing Program Analysts (LPAs) Angelica Wallin and Jeanette Estrada, conducted an unannounced Annual Required Inspection for the preschool license. During today’s inspection, LPA met with teacher’s assistant, Maria Sanchez, and toured the facility indoors and outdoors. This facility consists of 2 classrooms and outdoor play area. Days and hours of operation for classroom K1 are 8:00 am – 2:30 pm, Monday through Friday. Days and hours of operation for classroom K2 are 8:00 am – 11:00 am for morning session and 11:35 am – 2:35 pm for afternoon session.

All areas identified on the Facility Sketch were inspected. LPAs observed classrooms. LPAs took census of 5 children in K1 classroom with enrollment of 9 students and a census of 6 students in K2 classroom with an enrollment of 12 students. LPAs observed 1 teacher, 1 teacher assistant and 1 substitute teacher present during time of inspection. Capacity and limitations as specified on the license are being maintained.

The following was observed during the tour of the facility: furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. Carbon monoxide detectors were observed and are operable. Children have their own cubby to store their belongings. Age-appropriate sinks and toilets were inspected for availability and good repair in all classrooms. General sanitation was observed.

Drinking water via water dispenser was observed in classrooms. Per PIN 21-21-CCP lead testing to be conducted for all equipment used for drinking consumption. Per lead teacher, no water fountains used for consumption of water. Record review regarding lead test results to be conducted at Early Childhood Education-State Preschool Office.

Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children.

Monthly menus are posted in the classroom. Per staff, breakfast and lunch food service is provided by school. LPAs observed school kitchen and cafeteria. All kitchen, food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. All food is protected from contamination. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. There is adequate shade in the play yard. LPAs advised that no children shall be left without the supervision of a teacher at any time. All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic free. There are no firearms stored on the premises. There are no pools or bodies of water at the facility.

Per children’s record review, LPAs observed 6 children’s files that all children’s files are maintained and updated and include the following documents: Admission Agreement, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights, LIC 613-Personal Rights, LIC 702-Health History, and LIC 701 Physicians Report, and Immunization Records.

Complete staff record review regarding the following documentation: mandated reporter certificate, LIC-501: Personnel Record, LIC 9052- Employee Rights, LIC 503- Health Screening, Proof of immunization against measles, pertussis and influenza or influenza declination, TB clearance, and LIC 9108- Statement Acknowledging Requirement to Report Child Abuseto be completed at Early Childhood Education-State Preschool Office.



LPAs informed staff that at least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. Per record review, LPAs observed that teacher’s assistant, Maria Sanchez, has updated CPR and first aid certificate with expiration of 3/24/2025. LPAs advised director that the name of the child care center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day.

Teacher staff was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center.

A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP
LPA referred licensee to the Department website for lead: Lead Toxicity Prevention and Water Testing Information.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. For IMS information see PIN 22-02-CCP. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice) or (800) 514-0383 (TTY) and link to publication. Per lead teacher, school nurse is the only staff member to administer prescription medication only with attached doctor’s note to children in care.

Teacher staff was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. Exit interview conducted and report was reviewed with teacher's assistant, Maria Sanchez. A notice of site visit was given and must remain posted for 30 days.
