An unannounced Required one year Inspection was conducted on this day by Licensing Program Analyst (LPA) Raul Navarro. Facility is currently licensed for a capacity of 46 School Age children. Licensing arrived at about 02:10 pm and met with Facility Representative Cherie Ellis Director Shannon Cheffen arrived at the end of the inspection. The program currently operates Monday thru Friday from 2:00pm to 6:00pm. LPA observed 18 children in care with four staff upon arrival.

LPA toured the School Age classrooms and outdoor play area. Furniture and equipment was inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Storage for children's belongings was observed. Availability of drinking water was reviewed. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. Materials for the children were observed to be clean.

A first aid kit is kept in the classroom. LPA Navarro did not observe any carbon monoxide detector during the inspection. Fire extinguishers have been serviced. Hazardous items including poisonous cleaning compounds were stored inaccessible to children.

Outdoor equipment was inspected for safety, cushioning material, good repair and appropriateness. Required shade, drinking water and fencing were inspected. LPA Navarro advised that the children need to be within the direct care and supervision, including visual supervision of the teachers at all times.

The children have access to drinking fountains during outside play, LPA also observed bottled water available for children in care. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Children bring their own food from school. School provides the children with a sack of lunch. Snacks are available upon request. Food preparation areas were toured for safety, cleanliness and proper equipment.

Sign-in and out procedures were reviewed. Staff and Children’s Records were reviewed. LPA did not observe immunization records during file review. Criminal Record Clearances were reviewed for adults. LPA observed that two staff were not associated to the facility. Per Director, staff has been fingerprint cleared through the city of Carson, however, they are not associated to the facility. There is at least one staff member present with a valid CPR/First Aid card. Inspection of other required forms was made.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The deficiencies listed on the following pages were observed by the LPA and are being cited in accordance with California Code of Regulations Title 22. Please see attached LIC 809-D. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

Facility Representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
