Licensing Program Analyst (LPA) Rita Ramos conducted an unannounced required annual inspection. LPA met with Maria Ruvalcaba, Head Teacher, who guided analyst on a tour of the facility. LPA was later met by Natasha Jackson, Early Learning Manager. This is a preschool program which consists of 3 classrooms; Room 1, Room 2 and Room 3. This facility operates Monday through Friday. Rooms 1 and 2 operate a full-day program from 7:45AM to 2:45PM and Room 3 operates 2 part day sessions. The AM session in Room 3 is from 7:45AM to 11:15AM and the PM session is from 12:15PM to 3:45PM.

All areas identified on this report were inspected. Upon arrival, the following staff were present during this inspection: Room 1: Staff #1 and #2 with 18 preschoolers; Room 2: Staff #3 and #4 with 16 preschoolers; Room 3: Staff #5 and #6 with 12 preschoolers. The following was observed during the tour of facility:

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Children in the full-day program nap at the facility, however, children in the the part day program do not nap at the facility. Linens are washed weekly by a laundry service. Napping equipment (cots) were observed to be stored for children in Rooms 1 and 2. Per Head Teacher, the isolation area is located in the office. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. Upon inspecting the hand washing sink in Room 1, LPA observed that the faucet has a leak. Per Head Teacher, a request to have it fixed has been placed. The Early Learning Center Manager provided a copy of the service request that was provided to the district to have the sink repaired. LPA observed that facility does have a carbon monoxide detector. General sanitation was observed. Availability of indoor drinking water was observed in classrooms.


Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. The Head Teacher states that there are no poisons stored at the facility. --- Page 1 of 3

Menus are posted one month in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Snacks were reviewed for availability, quantity and appropriateness to children in care. All meals and snacks are provided by the Long Beach Unified School District.

First Aid supplies were observed in the classroom in a cabinet. According to the Head Teacher medication is only administered to a child when accompanied with a doctor's note and is stored in each classroom in locked medication boxes.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPA advised the Early Learning Center Manager to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

There were no deficiencies cited during today’s inspection.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Exit interview was conducted with Natasha Jackson, Early Learning Center Manager, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.

This facility does not have a kitchen area since all the meals are brought to the facility from the nearby elementary school. The facility does however have a food storage area. LPA observed that the food storage area is kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. LPA Ramos advised the Head Teacher that the children need to be within the direct care and supervision, including visual supervision, of the teacher(s) and supervising staff at all times. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. The outdoor yard is has two sections. One section is on one side of Room 3 and the other section is one side of the office. LPA observed that there is adequate shade in the play yard section next to Room 3, however, when inspecting the section next to the office LPA did not observe adequate shade except for some shading provided by the fence along the edge of the outdoor yard. LPA advised the Head Teacher and the Early Learning Center Manager that there needs to be some permanent shade in case the fencing were to not provide the shade that it is currently providing. An outdoor patio umbrella was placed outdoors during the inspection. Availability of outdoor drinking water was observed.

All individuals present have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Long Beach Unified School District. There is at least one person trained in CPR and Pediatric First Aid present during this inspection.

Children’s Records were reviewed for completeness; Inspection of required forms was made. LPA issued the Children’s Record Review (LIC 857) to the Early Learning Center Manager during this inspection.

LPA also issued the Review of Staff records (LIC 859) to the Early Learning Center Manager during this inspection. The LIC 859 documents the staff files that were reviewed during this inspection.

Children's roster was reviewed and is current. Sign-in and Sign-out sheets were reviewed. Children present were signed-in. Disaster drill log was available, last drill was conducted on 10/17/19. ---Page 2 of 3
