Licensing Program Analysts (LPAs) Rita Ramos and Steven Tung conducted an unannounced Required 1 year inspection on 08/26/2021 at 8:47AM. LPAs met with Maria Ruvalcaba, Head Teacher, who guided analysts on a tour of the facility. This is a preschool program which consists of 3 classrooms; Rooms B-1, B-2, and B-3. Facility operation hours are Monday through Friday from 7:45AM to 2:45PM. This facility is located within Webster Elementary School in the city of Long Beach. Early Learning Managers, Natasha Jackson and Karen Ascencio arrived during the inspection.

All areas identified on this report were inspected. Upon arrival, the following staff were present during this inspection: Room B-3: Staff #1, #2 and #3 with 5 preschoolers. Rooms B-2 and B-3 did not have any children present due to the new school year for those two classrooms commencing 09/01/21. The facility was observed to be within the license capacity and limitations. The following was observed during the tour of the facility:

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Linens are washed weekly by a laundry service. Napping equipment (cots) were observed to be stored in the classroom. Per Head Teacher, the isolation area is located in the nurse's office. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Availability of indoor drinking water was observed in classrooms.


Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. While inspecting Room B-1, LPAs did advise Head Teacher to ensure that children are not able to pull any cleaning products from underneath the sink cabinet due to the cabinet door having a wide opening at the bottom. Per Head Teacher, there are no poisons stored at the facility. Carbon monoxide detectors were observed and are operable. -----Page 1 of 3

All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. During the inspection LPAs observed that trash cans used to discard food have tight fitting lids.

When inspecting the outdoor playground LPAs observed bricks and concrete tiles that were removed during the inspection. The playground equipment was observed to be in a safe condition, free of sharp, loose or pointed parts however, when walking on the yard to get to Rooms B-1 and B-2, LPAs observed that there are 3 rain gutters made of steel that have sharp and exposed edges. One of the water gutters had a worn down sponge cover. LPAs advised that the cover be replenished. The water gutters were covered during the inspection. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. When inspecting the yard behind Room B-3, LPAs observed that there is no shade. Head Teacher informed LPAs that the umbrella shade was recently stolen and provided LPAs with a copy of the order request for a new shade umbrella. The umbrella shade arrived and was placed during the inspection. The yard was not observed to be in use by the children present during the inspection. Children present were able to play an use the yard in between all the classrooms that was observed to have a shade. Availability of outdoor drinking water was observed via a water pitcher and disposable cups. The facility does have drinking faucets, however, due to COVID-19, the facility is providing disposable cups. LPAs advised that no children shall be left without the supervision of a teacher at any time.

All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic-free There are no firearms stored on the premises. There are no pools or bodies of water at the facility.

All individuals present have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Long Beach Unified School District. There is at least one person trained in CPR and Pediatric First Aid present during this inspection.

Children’s Records were reviewed. The name, address, and telephone number of child’s authorized representative are on file. LPAs also reviewed staff records. The review of Staff records was documented on the LIC 859. Staff present did have proof of the AB 1207 Mandated Reporter Training certificate on file. Staff present did have proof against TB, measles, pertussis, and influenza. All staff have been given on the-job training sanitation principles, housekeeping, including universal health precautions. ---Page 2 of 3

LPAs also issued the Review of Staff records (LIC 859) to the Early Learning Manager during this inspection. The LIC 857 and the LIC 859 documents the staff and children’s files that were reviewed during this inspection.

Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed. Children present were signed in. Menus are posted one month in advance. The facility has a parent board posted outside by the entrance for all parents to see when they drop-off. Menus for the past 30 days are available upon request.First Aid supplies were observed in the classroom in a cabinet. According to the Head Teacher, there is currently no children that is on medication. If the facility enrolls a child that requires medication, the medication will only be administered to a child when accompanied with a doctor's note and will be stored in the classrooms where they will be inaccessible to children in care.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPAs advised the Early Learning Manager to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov. LPAs provided the Head Teacher with a copy of the Effects of Lead Exposure form during the inspection and discussed the following Provider Information Notices (PINs) Required Lead Testing: PIN 20-01 CCP Effects of Lead Exposure, and Guardian User Account Access - PIN 20-20-CCLD.



There were no deficiencies cited during today’s inspection.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Exit interview was conducted with Early Learning Manager, Karen Ascencio, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
