25 | On 04/20/2023, Licensing Program Analyst, Adrian Risher conducted a case management inspection regarding an incident that occurred on 04/06/2023. LPA observed 20 children present with 4 staff.
On 04/06/2023, ESCCRO received a call from the Director regarding an Unusual Incident. Director stated that staff discovered a child outside in the playground without supervision. Staff acknowledged that the child was found outside in the play structure when the parent arrived to pick them up.
LPA observed staff providing supervision in the classroom, on the play ground and during snack time.
Based on the investigation which included interviews with relevant parties and observations by the LPA, the preponderance of evidence to prove there was a Lack of Supervision violation has been met. Child 1 was left outside on the playground unsupervised after staff and children came back inside of the school. Staff discovered the child outside upon the parent's arrival to pick her up. The facility is being cited on the attached LIC 9099D page. Please refer to LIC421M for the Civil Penalty Assessment due to Lack of Supervision.
LPA Risher informed Director Juli Beck that this report dated 04/20/2023 documents 1 Type A citation(s) which shall be posted for 30 consecutive days as there are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Risher informed the Director to provide a copy of this licensing report dated 04/20/2023 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
Exit interview was conducted and a copy of the report was provided. Appeal rights were reviewed and provided.
