Licensing Program Analyst (LPA) Doris Whitmore met with Licensee Eromie Kumarage for the purpose of a Annual Required Inspection. LPA Whitmore toured the facility, all identified rooms per facility sketch was inspected, the following was observed. Pre- school hours are from 7am to 6pm Monday through Friday. Facility is licensed for Preschool and School Aged Program; Preschool Program will be inspected today.

Upon arriving at the preschool LPA observed 23 children being supervised on the playground for morning activity by 3 teachers. A total of 22 children and 1 Director, and 3 Teachers were present during the inspection. There are 3 separate classrooms for instruction. Classroom #1 for ages 3 years old, Classroom #2 for ages 4-5 years old, and Classroom #3 for age 2 years old. LPA observed sign/sign out sheets. Facility is operating within capacity limitations. A walk through of each classroom was conducted, classrooms were found to be clean and free from any potential hazards. LPA observed classrooms to have dramatic play, arts/crafts, reading and learning activities.

Furniture was found to be in good repair and age appropriate. There is adequate heating, lighting and ventilation. Drinking water is readily available in each classroom. Director states that the isolation area for sick students is located in the Director's office on a cot or mat. Napping equipment and bedding was inspected for good condition, appropriate storage and cleanliness. Children nap in individual classroom.

The bathroom area(s) were inspected. There are sufficient toilets and sinks to accommodate the children in care. Toilets flush properly, toilet and sinks are reachable by the children in care. Each restroom has adequate Toilet paper and paper towels available. There is adequate lighting/ventilation in the bathroom area(s).

Lunches are not provided by the school; Parents provide lunch for their children. Cleaning supplies are out of reach to the children and inaccessible to children in care. Page 1

The licensee was advised of the requirement to report unusual incidents and/or injuries to the parent/guardian and licensing within the time frame specified by the regulation. The "Notification of Parent's Rights" (PUB394) poster must be posted in an area accessible to parents. The information regarding new legislation with regards to exemptions and Parent’s Rights was also discussed.The licensee was advised that the Notice of Site Visit must be posted at the entrance of the facility for a period of 30 days. If a serious violation is cited, a copy of the licensing report (LIC809 or LIC9099) must also be posted for 30days. If these requirements are not met, civil penalties in the amount of $100 per violation will be assessed. Copies of the reports must also be provided to each parent when a serious deficiency, Type A, is cited (LIC9224).

An appeal may be filed, in writing 15 business days from the date of receiving the penalty assessment. As of September 1, 2016, SB 792, prohibits a person from being employed or volunteering at a child care facility or family day care if he or she has not been immunized against influenza, pertussis and measles. LPA discussed the influenza waiver during the visit.



The licensee was informed of The Child Care Advocate Program (CCAP) that is administered from within the Community Care Licensing Division. CCAP participates in many community activities and special projects in order to disseminate information on the State’s licensing role, provide information to the public and parents on child care licensing, and provide many other helpful resources to the licensees and the public. AB 1207: Beginning on January 1, 2018, this law requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Inspection of the outdoor play area was conducted climbing structures, swings, slides and other large play equipment is found to be securely anchored with adequate resilient cushioning material underneath and around the perimeter. Drinking water is readily available on the play yard. There is adequate shade for the children in care. Fencing around the perimeter of the play area is at least 4 feet high. Playground is free from miscellaneous debris such as tree branches, trash, leaves, etc. First Aid supplies are stored in each classroom and in the Director's office. All medication is stored in Directors office. A record of teachers /assistant’s names was check against current facility roster; Staff are currently associated to the campus in which they are assigned to work. LPA observed CPR/First Aid expires two were already expired and LPA spoke to the director about renewing the First/ Aid and CPR and the importance. Director will email documentation upon completition.
The following was discussed with the licensee:
Mandatory Forms for the children’s files and provider’s files, Requirements for fire drills, earthquake drills and documentation for both. Role and responsibilities of being a mandated reporter were discussed. Reviewing Files Mandated Reporter Certificates were not current. LPA discussed with the Director it is due every two years. LPA shared the website Director will email certificates upon completion The licensee was advised how to access forms and Regulations for Family Child Care online at www.ccld.ca.gov . Licensee was made aware that it is their responsibility to know the regulations as well as anyone who assists in providing care. Licensee was advised on how to access quarterly reports, forms, and regulations for Child Care online at www.ccld.ca.gov. The licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. Licensing must have the facility’s phone number at all times; if the phone number is changed, licensing must be notified. Regulation prohibits the smoking of tobacco on the premises.

The ADA requires that child care providers not discriminate against persons with disabilities on the basis of disability, that is, that they provide children and parents with disabilities with an equal opportunity to participate in the child care center's programs and services.Requirements for fingerprint clearances and associations were discussed with the licensee. Licensee can be cited a civil penalty of $100 per day, up to $500.00 (5 days) for the 1st offense and up to $3000.00 for the 2nd offense within a 12 month period.

New Immunization Requirement: Law enacted by SB 277, beginning January 1, 2016, personal beliefs exemptions will no longer be an option for the vaccines that are currently required for entry into child care or school in California. Personal beliefs exemptions already on file will remain valid until the child reaches the next immunization checkpoint.
New Appeal Process: A licensee may file an appeal, in writing 15 business days from the date of receiving the penalty assessment.

Nutrition Requirement: Beginning January 1, 2016, AB 290 will require for each new license issued, at least one director or teacher at each child care center or family child care home to have at least one hour of training in the importance of childhood nutrition. This applies to anyone submitting a new application, relocating their facility, selling their facility or transferring their license. Please note this training cannot be completed online or by home study programs. The training must be taken from an Emergency Medical Services Authority (EMSA) approved training program OR an accredited college or university.

Update on Incidental Medical Services: Facilities that provide Incidental Medical Services (IMS) must identify those services in their facility’s Plan of Operation and submit an updated Plan of Operation to the Department. Incidental Medical Services Include: Blood-Glucose Monitoring for Diabetic Children, Administering Inhaled Medication, Administering EpiPen Jr. and EpiPen or other Epinephrine Auto-Injectors, Glucagon Administration, Gastrostomy Tube Care (G-tube care), Insulin Injections Administration, Anti-Seizure Administration, and Emptying an Ileostomy Bag. This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.
