Licensing Program Analyst (LPA) Rita Ramos conducted an unannounced required 1 year inspection to the above facility on 10/14/2021. LPA arrived at the facility at 12:15PM, identified self and initially met with Katrina Crandall, Teacher, who guided analyst on a tour of the facility. Melissa Allen, Head Teacher also met with LPA and also guided a tour of the facility. LPA provided Head Teacher with a copy of the LIC 125 Entrance Checklist to help facilitate the inspection. This is a preschool program which consists of 2 classrooms; Preschool Room and Early Head Start Room (EHS) . Facility operation hours are Monday to Friday from 7:45 AM to 2:45 PM. LPA was later met by Natasha Jackson, Early Learning Center Manager.

All areas identified were inspected. This facility has 2 floors and a basement. The upstairs has 5 rooms that are used as offices for teachers and home base staff and are inaccessible to children in care. There is also a large room located outdoors that is used as storage. LPA observed that it was inaccessible to children as well. The basement is locked and it is not used by staff or children. LPA inspected the upstairs and the basement to ensure inaccessibility. Upon arrival, the following staff were present during this inspection: Preschool Room: Staff #1, #3 and #4 with 7 preschoolers; EHS : Staff #2, #5 and #6 with 7 preschoolers. The facility was observed to be within the license capacity and limitations. The following was observed during the tour of the facility:

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Linens are washed weekly by a laundry service. Napping equipment (cots) were observed in the classrooms. The isolation area is located in the Head Teacher's office. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Availability of indoor drinking water was observed in classrooms.


Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children and locked in the custodial closet. ----Page 1 of 3

Head Teacher states that there are no poisons stored at the facility. Carbon monoxide detectors were observed and were tested to ensure that they are operable.

This facility has a kitchen, however, all food and meals are delivered from the districts main cafeteria. The kitchen was observed to be clean. All storage containers for solid waste, including moveable bins have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed. LPA advised that no children shall be left without the supervision of a teacher at any time. There is a gated door and fence that leads to the parking lot located on the yard. LPA observed that it was locked to ensure that children do not have access to the parking lot. There is also a garden area located outdoors with another room used as storage. The room was observed to be locked.

All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic-free There are no firearms stored on the premises. There are no pools or bodies of water at the facility.

All staff and employees have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Long Beach Unified School District.

There is at least one person trained in CPR and Pediatric First Aid present during this inspection.

Children’s Records were reviewed. Inspection of required forms was made and documented on the LIC 857.

LPA also reviewed staff records. The review of Staff records was documented on the LIC 859. Staff present did have proof of the AB 1207 Mandated Reporter Training certificate on file. Staff immunization records are stored at the administrative office. All staff have been given on the-job training sanitation principles, housekeeping, including universal health precautions. -------------Page 2 of 3

Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed. Children present were signed in. Disaster drill log was available, last drill was conducted on 10/12/21.

Menus were observed to be posted. Preschoolers are provided with a breakfast, lunch, and snack.

First Aid supplies were observed in the classrooms. According to the Head Teacher, currently there are no children on medication.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. There were no deficiencies cited during today’s inspection.

A notice of site visit was given and must remain posted for 30 days.
