Licensing Program Analyst (LPA) Rita Ramos conducted an unannounced annual inspection on 07/13//2021 at 1:35 PM. LPA initially met with Kimberly Kelly, Coordinating Teacher, who guided analyst on a tour of the facility. LPA was later met by Cristabel Rodriguez, Program Coordinator. This is a preschool program which consists of 4 classrooms; RM 35, RM 36, RM 30 and RM 31. Facility operation hours are Monday to Friday from 6:30 AM to 5:30 PM in RM 35 and RM 36. RM 30 and 31 operate 2 part time sessions. The AM session operates from 8:00AM to 11:00AM and the PM session operates from 12:00AM to 3:00PM. This facility is located next to Grant Elementary School.

All areas identified on the Facility Sketch were inspected. Upon arrival, the following staff were present during this inspection: Room 36: Staff #1 and #2 with 1 preschooler. The following was observed during the tour of the facility:

Furniture and equipment were inspected for age appropriateness and good repair. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Availability of indoor drinking water was observed in classrooms.

Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were observed to be inaccessible to children. Per Program Coordinator, there are no poisons stored at the facility. Carbon monoxide detectors were observed in each classroom but none were operable. LPA advised the Coordinating Teacher and Program Coordinator to replace the batteries on all carbon monoxide detectors.

This facility has a kitchen but food is not prepared in the kitchen. Food is brought in already individually packaged by a delivery truck during the summer and during the school year meals and snacks are carted in from the elementary school next door. ---Page 1 of 3

The kitchen serves as a food storage area. The food storage area was observed to be clean and are free of litter, rubbish, rodents, and/or any other vermin. Trash cans used to discard food have tight fitting lids.

When inspecting the outdoor playground equipment, LPA observed a mat that was dirty (pictures were taken). LPA observed that a play house/hut and play table were covered in dirt/dust and spider webs (pictures were taken). Per Coordinating Teacher, children in care do come out to the yard daily to play and the play structure is cleaned and wiped once a week. LPA advised the Program Coordinator to have the outdoor play equipment cleaned and wiped at least once a day and if possible more than once due to the current pandemic. LPA went over COVID-19 precautionary measures during the inspection. The Program Coordinator had staff clean and wipe the playground equipment during the inspection. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall.

Availability of outdoor drinking water was observed. LPA advised that no children shall be left without the supervision of a teacher at any time.

While inspecting Room 31, LPA observed that the floor was dirty and the ceiling had a removed ceiling tile with a wire hanging out of it and another ceiling tile had water damage (pictures were taken). There are no firearms stored on the premises. There are no pools or bodies of water at the facility.

All individuals present have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Long Beach Unified School District. There is at least one person trained in CPR and Pediatric First Aid present during this inspection.

Children’s Records were reviewed. The name, address, and telephone number of child’s authorized representative are on file.

LPA did not review staff files during the inspection due to the staff files being stored at the administrative office. LPA will review staff files at a later time and date. If staff files were to be incomplete during the time and date that LPA reviews files, a case management inspection will occur at the facility to reflect the incomplete files.

Children's roster was reviewed and is current. Menus are posted one month in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request.

According to Coordinating Teacher, medication is only administered to a child when accompanied with a doctor's note and is stored in a locked box in the classroom. The facility was observed to be equipped with an isolation area for any child who becomes ill and it is located in the office.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov. LPA provided the Program Coordinator with a copy of the Effects of Lead Exposure form during the inspection and discussed the following Provider Information Notices (PINs) Required Lead Testing: PIN 20-01 CCP Effects of Lead Exposure, and Guardian User Account Access - PIN 20-20-CCLD.



The following deficiency listed on the attached deficiencies page is being cited in accordance with California Code of Regulations Title 22.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Exit interview was conducted with Coordinating Teacher, Kimberly Kelly, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.

This requirement is not met as evidenced by LPA observing that the outdoor play equipment is dirty, dusty, and covered with spiderwebs. Room 31 was observed to have dirty floors and the ceiling tile has water damaged and one tile is removed with a wire hanging out. This poses a potential health and safety risk to children in care.
