Licensing Program Analyst (LPA) Susann Sanchez conducted an unannounced random inspection. LPA met with Deyanria Sanchez, Lead Teacher who guided analyst on tour of facility. This is a preschool program which consists of three classrooms; RM 110 Sunflowers, RM 114 Poppies, and RM 116 Dandelions . This facility is located on CSUDH Facility operationing hours are from Monday to Thursday from 7:30 AM to 5:30 PM and Fridays 7:30 to 3:00 PM.

All areas identified on the Facility Sketch were inspected. Children (16 children) from room 110 & 114 were outside in the yard with 4 staff members. Children (4 children) in room 116 were in the outside garden with 2 staff member.

Disinfectants, cleaning solutions, medication and other items that may pose a danger to children, were observed to be inaccessible and are kept in a locked closet in each classroom restroom. Per Lead Teacher states that there are no poisons in the facility. Furniture and equipment are in good condition, free of sharp, loose, or pointed parts. All toilets and hand washing sinks are safe, sanitary and are operating properly. All floors are clean and safe.

All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish and rodents and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids. Drinking water is readily available both indoors and outdoors. The facility was observed to be free of flies, other insects and rodents.



Napping equipment and bedding was inspected for good condition, appropriate storage and cleanliness. Storage for children's belongings was inspected. Facility washes all linens on Fridays and parents take children's blankets home to wash. There is Laundry equipment in the the adult restroom. This area is inaccessible to children.

Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard.

All individuals present have obtained a criminal record clearance or criminal record exemption. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. First Aids Kits are kept in every classroom and in the office. Sign-In and Sign-Out sheets were reviewed. Children present were signed in. Disaster drill log was available, last drill was conducted on 10/17/19. Fire Ex was last serviced on 10/17/2019.

In review of children’s records, files contain information including, but not limited to the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary. At this time it was discovered that two children's medications have expired. Staff files were reviewed at the facility for completeness and accuracy to ensure teachers qualifications, mandated reporter training and current. Inspection of required forms was made. LPA issued the Confidential Names List (LIC 811) to the licensee during this inspection. The Confidential Names List documents the staff and children’s files that were reviewed during this inspection.

Snack menus are posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days are sent to parents. Snacks were reviewed for availability, quantity and appropriateness to children in care. The facility provides breakfast, PM snack and lunch.

On this LPA was able to review records for Staff #1, 2, 3 ,4 & 5. At this time staff are missing there Mandated Reporter Training. LPA discussed requirements, including, mandate reporting training for staff which can be found at: http:/www.mandatedreporterca.com/training /training.htm. This is a new regulatory requirement beginning January 2018. All licensed providers, applicants, directors and employees are required to complete training as specified on the mandated reporter duties and to renew their training every two years. Volunteers are encouraged but not required to take the training.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Lead Teacher Ms. sanchez, Director, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
