On 1/11/2023 Licensing Program Analysts (LPA) Judy Laureano conducted unannounced Annual Required Inspection at 10400 Wilshire Blvd. Los Angeles, CA 90024. LPA was greeted by Sarah Klinger, facility director. LPA toured the facility indoors and outdoors and the following was observed:

Room 55- 13 children and 3 staff members
Room 57- 13 children and 2 staff members
Room 56- 6 children and 3 staff members
Room 54- 0 children and 1 staff member
Room 52- 12 children and 2 staff members
Room 76- 15 children and 4 staff members
Room 78- 8 children and 1 staff member
Room 75- 9 children and 1 staff member
Room 77- 12 children and 1 staff member

Toddler Classroom- Room 112- 1 child and 2 staff members
Toddler Classroom- Room 114- 0 children and 3 staff members (1/2 day program)
Toddler Classroom- Room 60- 7 children and 3 staff members

Facility currently operates Monday through Friday 7:45 a.m. to 5:30 pm. During today’s inspection, LPA observed children in classroom getting ready for nap time. The preschool serves 223 preschool children ages 2 years to 5 years old and 36 toddler age children ages 18 months to 36 months.

LPA toured and inspected the facility in accordance with the facility sketch. Teacher/child ratios are maintained, and adequate supervision is being provided during this visit.

There is no swimming pool or other bodies of water on the premises. Facility has an armed guard waiver issued on 1/18/2022. Arm guards do not have any interactions with the children in the facility.

Disinfectants and cleaning solutions are kept in each classroom in a locked cabinet making the content inaccessible to the children in care. Director confirmed facility has a designated cleaning person that comes and cleans before the facility opens and during the day and at night time a cleaning crew comes in. First Aid Kit was observed in each classroom. Medication is stored in a locked cabinet in the classroom and in the director’s office.

Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts.

Facility has a current outdoor waiver issued 3/8/2022.

The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. LPA observed shaded area in the outdoor space with age appropriate play equipment and toys. Areas around high climbing equipment have cushioning material to absorb falls. Teachers ensure playground and outdoor equipment is safe and free of hazard before children use the area. LPA observed director removed one children’s tricycle that needs repair- a handlebar cover replaced. Director agrees to make the item inaccessible until repairs are completed. LPA observed an indoor gross motor area to be used in place of outdoor play yard for the toddler component.

All toilets and handwashing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. Facility provides partial snacks and menus are sent out to families via online app- Seesaw that is used to communicate with families. Parent provide lunch and partial snacks- fruits and vegetables, facility provides a carb and/or protein. All food is protected against contamination and any contaminated food is discarded immediately. Drinking water is available both indoors and outdoors, filter water is provided in each classroom and children are encouraged to bring their refillable water bottle.

Facility has custodial staff responsible for daily cleaning and sanitizing an outside pest control vendor is contracted on an ongoing basis and comes to facility monthly to ensure facility is free of flies, insects and rodents. Teachers are responsible for the daily cleaning in between classroom activities.
No poisons were observed during the inspection.

Solid waste storage containers have tight-fitting covers and are in good repair. There is a working fire extinguisher, smoke detector, carbon monoxide detector and adequate heating and ventilation for safety and comfort.

Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption. Capacity and limitations as specified on the license are being maintained.

Director informed LPA facility is moving toward electronic filing system for both staff and children’s file. All documents are available for LPA’s review when needed. LPA reviewed 10 staff files and observed all files were complete. LPAs reviewed children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment.

LPA provided facility admin with a copy of LIC311A and LIC 125 to use as a guide to audit files and program.

The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. Facility uses an electronic sign in and out sheet- LPA was provided with a printout of today’s sign in/out sheet.

All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 12 children in care.

LPA discussed Safe Sleep Regulations as it pertains to the Toddler component- Safe to Sleep Campaign https://safetosleep.nichd.nih.gov/materials

Facility representative, Sarah Klinger, was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) currently being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. Information regarding Americans with Disability Act (ADA) can be obtained by contacting US Department of Justice toll free ADA Information line at (800) 514-0301(voice), (800) 514-0383 (TDD) and website link https://www.ada.gov/childqanda.htm.



LPA and facility representative discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
