12/1/2022 Licensing Program Analysts (LPA) Judy Laureano conducted an unannounced Annual Required Inspection at 7300 Hollywood Blvd., Los Angeles, CA 90046. LPA was greeted by Jessy Lichter Meret- facility director and Alana Dintzer. LPA toured the facility indoors and outdoors and observed the following:

Room 1- 7 children and 2 staff member
Room 2- 11 children and 2 staff member
Room 3- 13 children and 3 staff members
Room 4- 16 children and 3 staff members
Room 5- 9 children and 2 staff members
Room 6- 13 children and 2 staff member
Room 7- 15 children and 3 staff members
Room 8- 16 children and 3 staff members
Room 9- 9 children and 3 staff members
Room 10- 15 children and 2 staff members

Facility currently operates Monday through Friday 8:00 a.m to 5:00 p.m. The preschool serves children ages 2 years thru 5 year old.

LPAs toured and inspected the facility in accordance with the facility sketch. Teacher/child ratios are maintained, and adequate supervision is being provided during this visit:

There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Facility currently has a fire arm waiver on file issued on 5/6/2022.

Disinfectants and cleaning solutions are kept in each classroom, top cabinet making the content inaccessible to the children in care. First Aid Kit bags were observed in each classroom. Medication is stored in each classroom unless medication needs to be refrigerated. If medication needs to be refrigerated, medication is kept in the director’s office. Currently facility does not have anyone that needs medication refrigerated.

Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts.

The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. LPA observed different shaded area in the outdoor space with age appropriate play equipment and toys. Areas around high climbing equipment and slides have cushioning material to absorb falls. Maintenance and teacher ensure playground and outdoor equipment is safe and free of hazard before children use the area.

All toilets and handwashing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. Food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. Facility maintenance is responsible for daily cleaning and sanitizing after hours and an outside pest control vendor is contracted to ensure facility stays pest free. Facility is free of flies, insects and rodents. Teachers are responsible for the daily cleaning in between classroom activities. No poisons were observed during the inspection.

Facility provides morning and afternoon snack and menus are emailed to all parents. LPA discussed with director the importance of maintaining menus for at least 30 days and noting any changes and/or modification that were made.

All food is protected against contamination and any contaminated food is discarded immediately. Families are responsible for providing lunch. Facility does offer a hot lunch and/or cold lunch program. Drinking water is available both indoors and outdoors, filter water is provided in each classroom and children are encourage to bring their refillable water bottle.

Solid waste storage containers have tight-fitting covers and are in good repair. There is a working fire extinguisher, smoke detector, carbon monoxide detector and adequate heating and ventilation for safety and comfort. Facility has a centralize Fire alarm with sprinkler system.

Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption. Capacity and limitations as specified on the license are being maintained.

At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. Director’s Preventative Health and Safety was completed on 11/4/2017 and CPR and First Aid was completed on 1/27/2021. All staff’s file is maintained with Human Resource. LPA discussed with director creating a system where Mandated Reporter Training and CPR and First Aid certification are available for LPA’s review.

LPA provided facility admin with a copy of LIC311A and LIC 125 to use as a guide to audit files and program.

The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. Facility uses an electronic sign in method for all parents. LPA reminded director that sign in sheets should be available for LPA’s review with all the required information as CCR 101229.1 requires.

All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 12 children in care.

LPAs reviewed children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment.

LPA discussed the following with director: Current outdoor waiver issued to facility. LPA will follow up to ensure all documents are on file and possibly having facility submit documentation to ESCRRO. Toddler option being added to the current preschool license- possible outdoor space was observed.

Facility representative, was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) currently being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. Information regarding Americans with Disability Act (ADA) can be obtained by contacting US Department of Justice toll free ADA Information line at (800) 514-0301(voice), (800) 514-0383 (TDD) and website link https://www.ada.gov/childqanda.htm.



LPAs and facility representative discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
