A subsequent Case Management - Incident on-site inspection was made by Licensing Program Analyst (LPA) Thelma Razo. LPA met with Center Director Adriana Duran. LPA stated the purpose of the visit was due to Unusual Incident that happened on 5/5/2021. According to the reported incident, Parent #1 (P1) informed Director via email on 5/5/2021 at 9:13PM that Child #1 (C1) touched Child #2 (C2) inappropriately.

On 5/13/2021, LPA conducted an initial tele-inspection with Ms. Duran and together, toured the facility to include the playground, one bathroom and 3 classrooms. Interviews were conducted with Ms. Duran, Staff #1 and Staff #2 at the time of the tele-inspection. Parent #1 and Parent #2 were interviewed over the phone on 5/13/2021 and 5/7/2021 respectively.

Today, Director Duran guided LPA to a tour of the facility. LPA observed the bathroom is separate from the 3 classrooms and has two exits, one to the playground and another to the hallway leading to the classrooms. LPA re-interviewed C1 and was unable to interview C2 since C2 is no longer enrolled at this center. LPA previously attempted to interview C2 but parents declined. During this visit, LPA also interviewed Child #3 and Child #4. LPA observed 31 children with 6 staff which is in compliance with mandated staff-child ratio.

Various interviews have indicated that children are being accompanied to the bathroom. Per interviews, children did not touch another child inappropriately and there are no witnesses to validate the alleged inappropriate touching of C1 with C2. Based on interviews, information gathered and observation, facility was not cited under California Code of Regulations, Title 22, Division 12.



The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Facility Evaluation Report and Appeal Rights explained and provided to Director Duran.
