On 6/9/21 Licensing Program Analyst (LPA) Jeanette Estrada conducted an unannounced complaint inspection to the above facility. LPA met with Ana Tovalin, Director and went over the findings. At the time of arrival there were 8 children present with 2 staff.
During the course of the investigation, LPA conducted staff interviews, parent interviews and obtained additional supporting documents which determined that the allegations above have been substantiated. LPA reviewed documents on 5/18/21 and 6/9/21 which indicated that on 4/21/21 the facility was over ratio by one child between the hours of 6:30AM to 8:00AM. On 4/21/21 there were 5 children present with Staff #3.
Documents also revealed that the facility has allowed commingling between the Licensed components. Approximately 4 children enrolled in the Infant license have been placed in the two-year old classroom. Interviews were conducted which also corroborated the allegations. Based on interviews and review of documents, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. *****CONTINUED ON PAGE 2*****

2 Type A California Code of Regulations,(Title 22, Division 12 & Chapter Number 1), are being cited on the attached LIC 9099D.
The notice of site visit was posted where the parent/guardian of children enter and exit the facility. A copy of this report shall also be posted where the parent/guardian of children enter and exit the facility. Both the notice of site visit and licensing report shall remain posted for 30 consecutive days. Failure to maintain posting as required will result in a $100.00 civil penalty. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon their return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled child for the next 12 months. A signed Acknowledgement of Receipt (LIC9224) shall be in each child’s file, acknowledging receipt.

Exit interview conducted with the Director Ana Tovalin, during which appeal rights were explained. A copy of the appeal rights (LIC9058 01/16) were provided. The Director’s signature on this report acknowledges receipt of her rights.
