On February 3, 2023 at 9:00am, Licensing Program Analyst (LPA) Dayna Chambers conducted an unannounced complaint inspection to the above facility for the purpose of delivering findings for the allegations above. LPA met with Ana Fragoso-Tovalin, Director. LPA toured the facility with Director at 9:05AM. At the time of arrival there were 11 children present with 2 staff and Director; at the same time another staff member was rotating into the classroom to relieve Director.
During the course of the investigation, LPA conducted 26 interviews with parents and staff. Witness Observations and Statements revealed that the center operates out of ratio at various times during hours of operation. Additional information disclosed by witnesses stated that in order to maintain a daily ratio, children are moved from their assigned classrooms and/or moved to the office. Witnesses also stated that during arrival hours, the children are out of ratio because there is not enough staff coverage which results in comingling. LPA learned through disclosures from witnesses, the center does not consistently maintain enough staff to cover overlap for different shifts during operation hours of 6:30AM and closing time of
