32 | Toilets flush and faucets work. Toilets and sinks are age-appropriate. Climbing structures, swings, slides have adequate resilient cushioning material underneath and around perimeter. The outdoor play area has fencing at least 4 feet high around perimeter. Play equipment is developmentally appropriate for age group of children, 2-5 years of age. There are no bodies of water in the facility.
The following plan of correction items were discussed with facility staff Merav Cohen, Claire Peikon and Shana Fishman:
- Room 11 - Floor broken and lifted, which can be a tripping hazard for children in care.
- Room 11 - Sink in classroom is leaking.
- All outlets in accessible areas to children should be equipped with safety covers.
- Room 8 - has access to apartment and/or office space not noted on the facility sketch. Per Ms. Fishman, the space is not apart of the Maimonides Academy business address. Maimonides Academy to submit parcel and/or county assessor information regarding the lot information.
- Food service area is not clean or free of hazards. LPAs Denise and Claudia observed a rotted food in the refrigerator, broken trash can without a lid, black spots on the ceiling resembling water damage, damaged ceiling with holes, dusty food preparation areas and dirty floors. LPAs Denise and Claudia did not observe a sink in this area.
- Classrooms numbers should be properly labeled.
- LPAs Denise and Claudia observed dead bugs/insects in classrooms. Per Ms. Fishman, pest control services were scheduled for next day, 06/18/2020. LPAs Denise and Claudia requested a receipt of pest control service to be submitted.
Due to the COVID-19 pandemic and local and state ordinances, facility staff were directed to the CCLD.CA.GOV website for important updates and PINS. Facility staff were directed to PIN 20-06 regarding social distancing and healthy practices to follow.
An exit interview was conducted with the above items discussed. A copy of this report was emailed to Merav Cohen with instruction to reply to the email, which will serve as a signature. Final license determination will be made upon review by the Licensing Program Manager and correction of the above issues. A second prelicensing inspection will be scheduled to review the plans of correction. |
