On 07/21/2020, Licensing Program Analysts (LPAs) Denise Do Amaral Miranda and Claudia Escobedo met with facility staff Merav Cohen, Claire Peikon and Shana Fishman. The purpose of the visit was an announced second pre-licensing inspection at the facility to ensure that health, safety and personal rights as required by Title 22 Regulations governing California Child Care Centers will be met. No children were present during this inspection, as the facility has been closed since March 16, 2020 due to COVID-19 precautions. This is an application to increase capacity from 125 to 169 preschool children. The child care center will operate Monday-Thursday 8:00 a.m. to 4:30 p.m. and Friday 8:00 a.m. to 2:00 p.m.

LPAs Miranda and Escobedo toured the indoor and outdoor areas of the child care facility and observed that all outdoor and indoor passageways, stairways, inclines, ramps, open porches and other areas of potential hazard are kept free of obstruction. The child care center was clean and in good repair. Floors of all rooms have a surface that is safe and clean. Furniture and equipment was in good condition, free of sharp, loose or pointed parts. Play equipment and materials are age-appropriate. LPAs Miranda and Escobedo observed that each child will have accesss to individual storage space for his/her clothing, personal belongings and/or bedding. Director, Claire Peikon, states that pitchers of water are made available for children in the classrooms. LPAs Miranda and Escobedo observed mats that are used by children for napping. The mats were observed to be in a safe condition with no exposed foam. Toilets and hand-washing facilities were observed to be in a safe and sanitary operating condition. The outdoor play areas are equipped with shaded rest areas for the children. The outdoor play areas are free of hazards. The food service area was clean and free of litter and rubbish. The facility offers a midmorning and midafternoon snack. Lunch is provided by Schwartz Bakery.

The facility has first-aid kits available in every classroom, containing the following items: sterile first-aid

dressings, bandages, adhesive tape, scissors, and antiseptic solution. The facility office has tweezers and thermometers available. The facility has an isolation room located outside of Room 11. The isolation room isequipped with a mat that can be used by the ill child. A teacher or the Director will supervise the ill child, until he/she is picked up his/her parents/guardians.

LPAs Miranda and Escobedo observed ovens in Rooms 11 and 14. The facility is equipped with a fire alarm. The facility has the following fire extinguishers:
  • Room 8 - 3-A:40-B:C
  • Room 11 - 4-A:60-B:C
  • Kitchen - Type A Size II B:C

The facility currently has a written sign-in/sign-out procedure. Parents drop-off and pick-up children at the entrance of the child care center, where they sign-in or sign-out the child. At drop-off a teacher escorts each child to their classroom. Due to COVID-19 precautions, the facility will test each child's temperature before allowing them to enter the facility.

The following are posted in the the child care office: Current child care license, LIC 613A - Personal Rights Child Care Centers, menus, LIC 610 - Emergency Disaster Plan for Child Care Centers.

The following plans of correction from the prelicensing visit dated 06/17/2020 were observed by LPAs Miranda and Escobedo:
  • Room 11 - Floor broken and lifted, which can be a tripping hazard for children in care. - On 07/21/2020, LPAs Miranda and Escobedo observed the floor was fixed
  • Room 11 - Sink in classroom is leaking. - On 07/21/2020, LPAs Miranda and Escobedo observed that the leaking sink was fixed.
  • All outlets in accessible areas to children should be equipped with safety covers. - On 07/21/2020, LPAs Miranda and Escobedo observed outlets to be equipped with safety covers.
  • Room 8 - has access to apartment and/or office space not noted on the facility sketch. Per Ms. Fishman, the space is not apart of the Maimonides Academy business address. Maimonides Academy to submit

    parcel and/or county assessor information regarding the lot information. - On 07/21/2020, LPAs Miranda and Escobedo observed Room 8 now has no access to the other property. The door leading to the other property was removed and replaced with a wall. Room 8 has 2 emergency exits.
  • Food service area is not clean or free of hazards. LPAs Denise and Claudia observed a rotted food in the refrigerator, broken trash can without a lid, black spots on the ceiling resembling water damage, damaged ceiling with holes, dusty food preparation areas and dirty floors. LPAs Denise and Claudia did not observe a sink in this area. - On 07/21/2020, LPAs Miranda and Escobedo observed that the kitchen was clean and free of litter. The black spots on the ceiling were no longer visible. LPAs observed a working sink.
  • Classrooms numbers should be properly labeled. - On 07/21/2020, LPAs Miranda and Escobedo observed that the classrooms had room numbers as listed on the facility sketch.
  • LPAs Denise and Claudia observed dead bugs/insects in classrooms. Per Ms. Fishman, pest control services were scheduled for next day, 06/18/2020. LPAs Denise and Claudia requested a receipt of pest control service to be submitted. - On 07/21/2020, LPAs Miranda and Escobedo did not observe any insects or rodents at the facility.


The following plans of correction items were discussed with facility staff Merav Cohen, Claire Peikon and Shana Fishman:
  • The trash can bin in the kitchen should be equipped with a lid.
  • The child care center should be equipped carbon monoxide detectors.
  • The facility to inform Community Care Licensing when they transition to electronic sign-in/sign-out procedures.
  • PUB 393 - Notification of Parents' Rights needs to include the El Segundo Child Care Office contact information.
  • LIC 9148 - Earthquake Preparedness Checklist needs to posted.
  • Room 4 - Repair the ceiling, as it currently has a 4 inch in diameter brown spot
  • Room 5 - Broken ceiling to be repaired.
  • Earthquake emergency equipment to be readily and easily accessible to staff in case of an emergency.

LPAs Miranda and Escobedo provided the facility with the LIC 311A - Records to be Maintained at the Facility.

Due to the COVID-19 pandemic and local and state ordinances, facility staff were directed to the CCLD.CA.GOV website for important updates and PINS.

An exit interview was conducted with the above items discussed. A copy of this report was emailed to Merav Cohen with instruction to reply to the email, which will serve as a signature. Final license determination will be made upon review by the Licensing Program Manager and correction of the above issues.
