Licensing Program Analyst (LPA) Shandra Powell conducted an unannounced annual inspection on 11/22/22. A copy of the Entrance Checklist for Child Care Centers form (LIC 125) was provided to the Director upon entry. LPA viewed documents to be posted in a publicly area and reviewed sign in and out sheets. During the review of sign in and out sheets LPA observed one infant not signed in for the day. This poses a health and safety risk to children in care. Director, Massengale guided analyst on a tour of the facility. This is an infant program which consists of 1 classroom; Infant Room. Facility operation hours are Monday to Friday from 6:00 AM to 6:00 PM.

The infant program is located on the same grounds to the center's pre-school program #191870376 and the school-age program #197408576. The infant program has no shared areas. Upon arrival, the following staff were present during this inspection: Staff #1, #2 and #3 with 11 Infant Children in care. The facility was observed to be within the license capacity and limitations. The following was observed during the tour of the facility:

Furniture and equipment were inspected for age appropriateness and good repair. LPA observed age appropriate toys in the classroom. The facility has sufficient napping equipment, feeding chairs, and changing table. Cribs were observed to be free from loose articles and objects including blankets, pillows and no objects were observed to be hanging above or to the side of the cribs. LPA advised Director to have tight fitted sheets in all cribs. The facility has a separate napping area. LPA advised that no infants are to be swaddled. LPA provided the Director with a copy of A Child Care Provider’s Guide to Safe Sleep, by American Academy of Pediatrics and consulted about Sids, and Safe Sleeping practices.

Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Linens are taken home each week to be washed. The isolation area is located in the office. LPA observed a cot for ill children to rest on in office. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Availability of indoor drinking water was observed in classroom. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Smoke and Carbon monoxide detectors were observed.

All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids. Menu was observed to be posted in the classroom, front office and outside on the informational board. LPA advised that the menu be posted in an area in which parents can see. Food in refrigerator was labeled. Infant formula is available. Kitchen counter tops were clean and free of clutter or standing food.



The outdoor playground was observed to be physically separate from the preschool and school-age components. Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed. LPA advised that no children shall be left without the supervision of a teacher at any time. All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic free. There are no firearms stored on the premises. There are no pools or bodies of water at the facility.

Staff present has obtained a criminal record clearance or criminal record exemption. LPA observed at least one Staff present has training in CPR and Pediatric First Aid present during this inspection.

Children’s Records were reviewed. Infant Needs and Service plans were reviewed and observed to be in a file cabinet. However, LPA did not observe the LIC 9227 Individual Infant Sleeping Plan for the three infants enrolled under 12 months. LPA advised staff that infants need to be physically checked when sleeping every 15 minutes and document whether there is labored breathing, signs of distress, and sleep position (on their backs). The name, address, telephone of child’s authorized representative, and medical assessment where on file. Inspection of required forms was made and documented on the LIC 857.

LPA also reviewed staff records. The review of Staff records was documented on the LIC 859. All individuals present have obtained a criminal record clearance or criminal record exemption. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. Staff Records were reviewed to ensure that a health screening report is on file. Staff present did not have proof of renewal of the AB 1207 Mandated Reporter Training certificate on file. Staff present did have proof against TB, measles, pertussis, and influenza. All staff have been given on the-job training sanitation principles, housekeeping, including universal health precautions.



Children's roster was reviewed LPA reviewed 16 infants enrolled. Daily Activity Schedule Verification of Disasters and Fire Drills, Sign in/out Sheet and Earthquake Preparednes (LIC 9148) documentation reviewed during inspection and is current.

First Aid supplies were observed in the classroom on top of kitchen shelf. According to Director, medication is only administered to a child when accompanied with a doctor's note and is stored in the office. LPA observed a Fire Extinguisher located on wall in classroom with service tag dated August 2022. There are no children currently on medication.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

U.S. CONSUMER PRODUCT SAFETY COMMISSION FISHER-PRICE INFANT EQUIPMENT RECALLS: PIN 20-19 ,

During this inspection, LPA also provided the following documents about SIDS. 1) A Child Care Provider’s Guide to Safe Sleep by the American Academy of Pediatrics, 2) Safe Sleep for Your Baby by the U.S. Department of Health and Human Services.

Technical violations were provided during inspection to protect the children's health and safety.



The Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
