25 | Licensing Program Analyst (LPA) Judy Mora conducted a case management inspection due to an incident that occurred on 01/22/2020. LPA met with Director, Laura Drew. LPA conducted interviews and obtained documentation during this visit.
The incident that occurred on Wednesday, 01/22/2020, was reported to the Department on Monday, 01/27/2020, via fax. The Director was not aware that she needed to report this incident to the Department. The facility did not report the incident within the required 24 hours. The facility is being cited for not reporting this incident in a timely manner.
Based on the information provided and obtained during this visit, it was found that staff #4 pushed down on child #1's back in a forceful manner in attempt to make the child lay down for nap. Staff #4 violated child #1's personal rights. Disclosures were made in regards to the above incident.
Upon receipt of this report, the licensee shall post ANY licensing report documenting a type “A” citation. This must remain posted for 30 days during hours of operation. In addition to posting this report, the licensee will also provide copies to the parents of the children in care for up to one year.
A copy of the the LIC 9224 - Acknowledgement of Receipt of Licensing Reports was issued to the Director.
