25 | Licensing Program Analyst (LPA) Veronica Martinez Garza conducted an unannounced Case Management Incident inspection at the above facility on 01/18/24 at 1:23 p.m. The purpose of this inspection is to follow up on an incident reported to the Department on 01/09/24. LPA met with Director, Laura Drew who guided LPA on a tour of the facility. Census was taken.
On 01/08/24 at approximately 04:20 p.m. Child #1 (C1) was playing tag with C2 and other children, when C1 tripped and fell in the dirt triangle area (LPA took pictures). C1 tripped and hit their chin on the concrete edging of the dirt triangle and sustained an injury. C1 required medical attention and received 2 stitches. The facility provided first aid and called parents of C1. C1 has since returned to the facility and the only restrictions are to keep the chin area clean and dry. Per S1, the dirt triangle area is designated for children to play. LPA did not observe any hazards in the dirt triangle area or on the outside. The incident happened so fast that it couldn't have been prevented.
During this inspection, LPA interviewed,Staff 1 (S1), Child #1 and #2. Director provided LPA with a copy of the children's roster and the ouch report. LPA will need to interview S2 who witnessed C1 fall. A follow up visit will be required at a later date.
At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today.
The Notice of Site Visit (LIC 9213) must remain posted for 30 days during the hours of operation after each site visit by a licensing representative, a civil penalty of $100 can be assessed.
Exit Interview was conducted and appeal rights were given, along with a copy of this report was provided to the Director, Laura Drew.
