25 | Licensing Program Analyst (LPA) Justin Dorsey conducted an unannounced Case Management Visit. Due to COVID-19 preventative measures, the visit was conducted via Zoom on this date. LPA met with Veronica Montano Sanchez, supervisor whom the purpose of this tele-visit was explained, and guided LPA on a tour of the facility.
The purpose of this visit is to follow up on an incident reported to the Department on 07/28/20 via email. The Unusual Incident/ Injury Report was reviewed, stating: On 07/27/20 while Child #1 was taking a bath at home they told Parent #1 that Teacher #1 inappropriately touched them on the breasts and vagina while rendering first aid to an injury. On 07/28/20, Parent #1 reported the incident to the facility, who then reported the incident and began an internal investigation. The incident was sent to the Department's Investigation Bureau (IB) on 07/30/20 and was accepted as a full investigation assigned to Investigator Dennis Douglas.
During IB's investigation, facility staff (including Staff #1) and children were interviewed. Investigator Douglas also made contact with Detective #1 from the Los Angeles Police Department Abused Child Unit. After interviews and concluding the investigation, Investigator Douglas found the incident to be unsubstantiated.
Based on IB interviews and observations, there were no violations of Title 22 requirements.
An exit phone interview was conducted with supervisor Veronica Montano Sanchez and a copy of this report was signed by LPA Dorsey. This report along with the LIC 9213 will be sent via email to supervisor Veronica Montano Sanchez who agrees to sign and date the report. This report was sent via email and an electronic read receipt confirms receiving the report. The facility representative was provided with the mailing address for the Monterey Park Regional office and agrees to send the original report by mail. |
