Licensing Program Analysts (LPAs) Roxana Lopez and Mireya Garcia conducted an unannounced Annual Required Inspection. LPAs met with Site Supervisor, Veronica Montano Sanchez, who guided LPAs on a tour of the facility indoors and outdoors. This facility operates Monday-Friday from 6:30am-8:00pm. Per Site Supervisor, this facility has 50 preschoolers and 15 toddlers enrolled. This facility has a capacity of 89 children. There are 43 children present with 8 staff. Teacher-child ratio is met. There is also an infant center on site with license # 191806530. All adults present have obtained a criminal record clearance or exemption on file.

All areas used by the children were inspected. This program consists of 5 classrooms: Toddler 1; Room D-121A, Toddler 2; Room D-125A, Preschool 1; Room D-122A, Preschool 2; Room D-123A and Pre-K; Room D-124A. Upon arrival, LPAs observed 6 toddlers with 3 staff in the toddler classroom #D125A; 37 preschoolers with 6 staff in the preschoolers’ playground. Please note that all playgrounds are physically separate by a fence from the toddler’s playground. Teacher-child ratios were observed to be in accordance with Title 22 regulations. Staff names were recorded. All children were observed to be under visual supervision of a teacher at all times. Report continues on next page 1 of 5.

LPAs observed the facility to be clean, safe, sanitary and in good repair. Furniture and equipment was inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. All toilets and hand washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. At this time, the office is used as an isolation area. There are chairs in the office. Parents are contacted immediately when children are determined to be ill and staff are ensuring that children with obvious symptoms of illness are not being accepted.

Snack menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. The facility provides Breakfast, AM snack, lunch, PM snack and Dinner Snack. All kitchen, food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. All food is protected from contamination, and LPAs inspected that any contaminated food is discarded immediately. There is drinking water available in all indoor classrooms and drinking containers / jugs are taken outdoors. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in an area inaccessible to children. Site Supervisor, Veronica Montano Sanchez states that there are no poisons stored at the facility.

Facility has one functioning carbon monoxide detector that meets statutory requirements. The facility representative takes measures to keep the facility free of flies, other insects and rodents.

Outdoor play equipment was observed to be in good condition, free of sharp, loose or pointed parts. Outdoor activity space surface is maintained in a safe condition as is free of hazards. The Site Supervisor states that there are no bodies of water on the premises and LPAs did not observe any bodies of water during this visit. Areas around and/or under climbing equipment, slides have poured in place rubber cushioning material to absorb a fall; climbing equipment are single level only. Site Supervisor states there are no weapons or firearms on the premises.



Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. Staff Records were reviewed to ensure that a health screening report is on file. Children’s Records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. Criminal Records Clearance for adults and verification of CPR/First Aid and health preventative practices documentation was reviewed.

SB792 (Immunization Requirements for Staff and Employees) was discussed with the facility representative.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. In review of medication LPAs observed that medication for Child #1 was expired (Pictures were taken). Veronica Montano Sanchez, Site Supervisor was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The following deficiency listed on the following pages was observed by the LPAs and are being cited in accordance with California Code of Regulations Title 22. Please see attached LIC 809D for deficiency that is being cited and needs to be cleared to protect the children’s health & safety.

Based on LPAs observation and records review of child's #1 medication was expired on 09/2021, the licensee did not comply with the section cited above by discarding of medication by 09/2021 which poses/posed a potential health, safety or personal rights risk to persons in care.
