25 | On August 17, 2023, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced case management inspection at the above facility. The purpose of the inspection is to follow up on three incidents that were reported timely to the department. One incident occurred on 05/23/2023, the second incident occurred on 06/02/2023 and the last incident occurred on 06/26/2023. A COVID-19 risk assessment was conducted prior to entering the facility. LPA met with Principal Jessica Robeles-Logan who guided LPA on a tour of the facility. LPA observed 49 children in care with 14 staff.
Brief summary of incident #1: On 05/23/2023, Child #1 (C1) informed Parent #1 (P1) that Staff #1 (S1) grabbed C1's arm. Per P1 there were no marks on C1's arm. C1 stated that he was yelling at Child #2 (C2) and that is why S1 grabbed him.
Brief summary of incident #2: On 06/02/2023 at approximately 2pm, Staff #2 (S2) was sweeping the green room and found a vitamin on the floor, vitamin was picked up. Staff #3 (S3) noticed C2 holding something in her hand and questioned C2. C2 was holding another vitamin. C2 stated she got the vitamin from her cubby.
Brief summary of incident #3: On 06/26/2023, at approximately 3:40pm, Child #3 (C3) was on the apparatus. C3 lost her footing and hurt herself between the legs on the apparatus bar near the stair case. S1 provided first aid to C3. Parent #2 (P2) took C3 to seek medical attention. C3 obtained a laceration from the incident and returned to school on 06/28/2023.
During the inspection LPA interviewed C1, S1-S4, obtained a copy of staff written statements, a copy of the facility roster, obtained pictures of apparatus and other relevant documents. |
