25 | On 2/4/25 LPA Licensing Program Analyst (LPA) Claudia Kam conducted a case management to follow up on an incident reported to the department on 12/17/24 and 1/29/25. On 12/17/24, an incident was reported to The Department where a child had medication in her personal belongings. The facility reported this incident to the Department within the required 24 hours and submitted written report within seven days. Upon arrival, LPA met with principle Dr. La Royce Murphy who provided LPA a tour of the facility. LPA observed proper care and supervision. During this inspection LPA interviewed Facility Representative. She stated that interim priciple followed LAUSD protocols in calling Law Enforcement Agency (LAPD), submitted ISTAR report, and informed LAUSD Operations. LAPD notified Facility Representative to handle Administratively, and no investigation was conducted by them. Facility Representative states that an internal investigation is pending. Child affected is currently still attending care. LPA requested Facility Representative to provide update to Department should one become available.
During this inspection LPA interviewed staff, observed room incident took place in. LPA was unable to observe child named in report, as child was not in care at time of visit. Teacher informed Licensing that a child was found to have medication in her pockets at pick up, discovered by guardian. Child did not ingest the medication. Parent was informed and conference conducted with acting principle. Safety protocol training conducted with staff.
For incident reported on 1/29/25 Facility Representative states that they are conducting their own internal investigations regarding the reported incident which includes interviews of parents and children and obtaining written statements from Staff. The internal investigation is pending. LPA was unable to complete staff interviews as staff were not present. Due to insufficient information being available at this time, a follow up visit will be required at a later date. In the mean time, LPA requested Facility Representative to provide regular updates on course of investigation and to provide necessary documentation to LPA via email.
