13 | Licensing Program Analyst (LPA), T. Tran conducted an unannounced subsequent complaint inspection for the purpose of concluding the investigation of the above allegations. Upon arrival, LPA met with Emily Ramirez, Office Manager.
Based upon the evidence obtained during the investigation through interviews, and record reviewed, on 06/04/2021, the facility had reported to the licensing department regarding the facility restrooms were not working and plumber was contacted. Per staff, while pending for the portable potty to arrive, the facility had followed the emergency protocol by temporary setting up the emergency portable restrooms for children to use. On that same day, all parents were verbally informed upon pick up. On 06/07/2021, before the children arrived, three portable potties were arrived and set up by the staff parking area readily for children to use. On 06/08/2021, the facility restrooms were fixed, and the Environmental Health and Safety had completed the analytical test and cleared the facility restrooms usage. During this process, children and staff were accorded safe, healthful and comfortable accommodations therefore, the evidence does not support, nor disprove the above allegations were violated and have been determined unsubstantiated. Unsubstantiated – A finding that the complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. The copy of this report was explained and provided to the noted person along with appeal rights.
