13 | This was a complaint inspection conducted by Denise Gibbs, Licensing Program Analyst (LPA) on 4/19/22 at 11:30AM. This complaint was conducted with Elizabeth Mares, Facility Representative (FR) for the purpose of delivering findings.
There were 40 children and 20 staff present when the visit began.
During the course of this investigation, LPA conducted interviews. All pertinent documentation was collected. LPA observed that the the FR submitted required Unusual Incident form, LIC624, in regard to the current floor remodel on 3/23/22, before remodel began. During annaul insection on 3/23/22 LPA observed classroom 3 being cleared in preperation for remodel. The Unusual Incident report discusssed a plan for children during remodel. LPA observed a memo sent out via BlackBoard Connect to all famiies informing them of the remodel, sent out on 3/18/22. LPA also observed a calendar for April documented with daily remodel plans.
