On 06/11/2024 at 9:15 am, Licensing Program Analysts (LPAs) Roxana Lopez and Saul Valenzuela conducted an unannounced required inspection. LPAs met with Principal, Rafaela Campos, who guided the LPAs on a tour of the facility. This is an LAUSD state preschool program located across the street from El Sereno Elementary School. This program is licensed for 114 children which operates Monday – Friday from 7:00 AM – 5:30 PM. Per the Principal there are 96 children enrolled.

All areas identified on the Facility Sketch were inspected. The following staff was present during this inspection: Room 1A: Staff #1, Staff #2, Staff#3 and Staff #4 with 18 children; Room 1B: Staff #5, Staff #6 and Staff #7 with 19 children, Room 2: Staff #8, Staff #9 and Staff #10 with 20 children; Room 3: Staff #11, Staff #12, Staff #13 and with 19 children; Teacher-child ratios were observed to be in accordance with Title 22 Regulations. Staff names were recorded. All children were observed to be under supervision, including visual supervision, of a teacher always. The licensed facility is within the conditions, limitations, and capacity specified on the license.

LPAs observed the facility to be clean, sanitary and in good repair. Furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. All toilets and washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. Currently, the staff lounge is used as an isolation area. There are chairs and cots in the lounge. Parents are contacted immediately when children are determined to be ill, and staff are ensuring that children with obvious symptoms of illness are not being accepted.

The facility provides breakfast, lunch, and snack. Menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. Per Principal meals are dropped of daily by the cafeteria located across the street in the El Sereno Elementary. All food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. All food is protected from contamination, and LPAs inspected that any contaminated food is discarded immediately. There is drinking water available in the classroom via individual water bottles and water fountains- which are taken outside for children to drink. LPAs observed trash cans used for solid waste do have a tight-fitting lid. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in an area inaccessible to children. Per Principal that there are no poisons stored at the facility.

At 10:45 am LPAs observed that there are smoke and carbon detectors in the facility. The last Earthquake Drill was conducted on 5/24/2024 and Fire Drill on 5/8/2024.

LPAs observed sleeping arrangements in form of cots.

Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard.



Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day.

There is at least one person trained in CPR and Pediatric First Aid present during this inspection. All staff have received an active criminal record clearance as a condition of their employment with the Los Angeles Unified School District.

LPAs did not review complete staff files during the inspection due to the staff files being stored at the administrative office. LPAs will review staff files at a later time and date. If staff files were to be incomplete during the time and date that LPA reviews files, a case management inspection will occur at the facility to reflect the incomplete files. At 10:45 AM, LPAs observed that Staff #7 did not have a current proof of Mandated Reporter on file.

At 11:00 AM, Children’s Records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. .

AB1207 Mandated Child Abuse Reporting – On or before March 30, 2018 any person who works in a child care facility shall complete the training and renew the training every 2 years. Website provided: https://www.mandatedreporterca.com/training/child-care-providers.



This facility provides Incidental Medical Services – IMS. At 11:30 AM LPAs reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. C1 through C3 medications were not in their original packaging. LPAs observed that C1’s medication was expired.

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was completed in accordance with the Written Directives outlined in PIN 21-21.1-CCP.

MyChildCarePlan.org – Centers and Family Child Care Homes
facility representative was informed of the MyChildCarePlan.org website; a
consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Based on the LPA’s observations and records review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.



A notice of site visit was given and must remain posted for 30 days.
