Licensing Program Analyst (LPA) Rita Ramos conducted an unannounced complaint inspection on 07/09/21 at 08:50AM. The purpose of the inspection is to investigate the above allegations. LPA initally met with Fedra Chang, Teacher, who guided analyst on a tour of the facility. LPA was later met by De'Onna Phillips, Principal. There were 10 children and 10 staff present upon arrival.

During the investigation LPA obtained copies of the children’s roster, staff roster, and other supporting documents. LPA took pictures and conducted interviews with parents, staff, and children.

Information provided by the reporting party indicates that the facility is not cleaning, sanitizing, and/or disinfecting the outdoor playground equipment and there is no social distancing.

Principal states that the facility is cleaned inside and outside on a daily basis and chldren and staff are practicing social distacning.

Staff #1 and #2 stated that the facility is cleaned and disinfected on a daily basis several times a day. Staff #3 stated that the facility and staff promote social distancing.

Parent #2 stated that they have observed cleaning and disinfecting outdoors and on the playground. Parent #3 disclosed observing social distancing and cleaning at the facility. Parent #1 made no disclosures.

Child #1 disclosed seeing staff clean the facility and knows to stand on the 6 foot markers located on the floor when waiting their turn to wash their hands. Child #2 knows to stand on the 6 foot markers located on the floor to wait their turn to wash their hands. Child #3 disclosed social distancing at the facility and observing staff cleaning the play structure. Child #4 disclosed observing staff cleaning.

LPA observed social distancing markers in the classrooms, in the hallways, and outdoors on the benches and tables. LPA also observed custodial staff cleaning and disinfecting the facility.

Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is unsubstantiated

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Exit interview was conducted with De'Onna Phillips, Principal, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
