Prior to entrance to the facility, LPA Mayra Rivera conducted a Covid 19 assessment and based on the licensee responses to the facility assessment questions, LPA Rivera determined safe to proceed. On Monday, July 26, 2021 at 9:14 AM, Licensing Program Analyst (LPA) Mayra Rivera entered the facility to conduct an unannounced annual inspection and met with Director Blanca Aldrete who guided LPA Rivera on a tour of the facility. Facility operation hours are Monday to Friday from 6:30 AM to 6:00 PM.

The following staff were present during this visit: Room A: Staff #1 and #2 with 9 children; Room B: Staff #3 and # 4 with 4 children; Room C is closed to keep enrollment number low due to Covid; Room D: staff # 5, #6 and #7 with eleven children.

At 9:10 AM, LPA Rivera entered Room B to inspect for safety, comfort, cleanliness, and ventilation. During the inspection LPA Rivera observed 4 children doing art work. The following staff were present during this visit: Staff #3, Staff #4, and teacher child ratio were observed. LPA observed the disinfectants, cleaning solutions, and other items that are dangerous to children, inaccessible to children on top of high shelves making it inaccessible to children to reach. LPA observed the furniture and equipment in good condition, age appropriate and free of sharp, loose, or pointed parts. LPA observed hand washing sink area safe, sanitary and operating properly and floors clean and safe.



At 9:20 AM, LPA Rivera entered Room A to inspect for safety, comfort, cleanliness, and ventilation. During the inspection LPA Rivera observed 6 children doing art work. Three children arrived later making it a total of nine children present. The following staff were present during this visit: Staff #1, Staff #2, and teacher child ratio were observed. LPA observed the disinfectants, cleaning solutions, and other items that are dangerous to children, inaccessible to children on top of high shelves making it inaccessible to children to reach. LPA observed the furniture and equipment in good condition, age appropriate and free of sharp, loose, or pointed parts. LPA observed hand washing sink area safe, sanitary and operating properly and floors clean and safe.

At 9: 26 AM, LPA Rivera entered Room D to inspect for safety, comfort, cleanliness, and ventilation. During the inspection LPA Rivera observed eleven children doing manipulates. The following staff were present during this visit: Staff #5, Staff #6, and #7 and teacher child ratio were observed. An hour later, another child arrived making it a total of twelve children present. LPA observed the disinfectants, cleaning solutions, and other items that are dangerous to children, inaccessible to children above high shelves making it inaccessible to children to reach. LPA observed the furniture and equipment in good condition, age appropriate and free of sharp, loose, or pointed parts. LPA observed hand washing sink area safe, sanitary and operating properly and floors clean and safe.

At 9:34 AM, LPA entered Room C, which is currently not being used. The center is keeping a low enrollment due to Covid.

At 9:40 AM, LPA Rivera entered the restroom and observed four toilets and four sinks, hand soap and paper towels. LPA observed the restroom to be in good condition. LPA observed the children's personal belongings stored in the hallway inside personal plastic bags hanging individually. LPA observed cots and per director, children daily bring in their cot sheet and blanket. LPA observed the food preparation area/kitchen and the food storage area clean and free of litter, rubbish and rodents and/or any other vermin. LPA observed inside each classroom and in the kitchen, trash cans to discard food and waste with tight fitting lids to be in good repair. Drinking water is readily available both indoors and outdoors. LPA also observed water jugs and disposable cups inside each classroom and outdoor play area. Facility provides breakfast, lunch, and snack. For ventilation LPA observed central AC/heater and room temperature at 72 degrees.

At 9:55 AM, LPA Rivera inspected the outdoor area for safety, comfort and cleanliness. The facility was observed to be free of flies, other insects and rodents .LPA observed the outdoor playground equipment in safe condition, free of sharp, lose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. LPA observed a sail shade triangle in place and provides adequate shade in the yard. LPA observed the playground structures to be age appropriate ages two to five and five to twelve. LPA observed the sandbox area to be closed off due to Covid.

Smoke detectors are present in the facility, but were not tested as they are hardwired through the building.
LPA observed the required 2A10BC fire extinguisher located in the classrooms, kitchen and the valve on the green area indicating fully charged and serviced in December 8, 2020. LPA observed first aid kids inside the classrooms cabinets and the earthquake and fire drill log posted on the bulletin board and classrooms. Last drill was conducted on June 28, 2021.

LPA observed and reviewed breakfast/lunch/snack menu Facility provides the meals and are brought from the main kitchen. The menus are posted in each classroom and the hallway. Menus for the past 30 days are available upon request.

LPA reviewed children’s records and issued Children's Record Review form (LIC 857) to Director during this inspection. Staff files were not reviewed on this date as they are housed at their main site Amistad Preschool (198008821). LPA reviewed director pediatric first aid/CPR certification. Certification expires May 2023.

The following was discussed with the Director:


Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm
LPA advised Director that all adults 18 years of age and older providing Care & Supervision and/or have continuous presence in the facility shall adhere to a criminal background clearance with the Department of Justice, FBI and Child Abuse Index Check.

Facility administers medication. Before administrating medication, parents sign a medication consent form, provides the medication in original container with Dr's prescription. At this time, facility has no children with medication or allergies.

LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

LPA guided Director in how to use Guardian. LPA explained the purpose of utilizing guardian and the procedure in how to associate staff to the facility.

LPA advised Director that all adults 18 years of age and older providing Care & Supervision and/or have continuous presence in the facility shall adhere to a criminal background clearance with the Department of Justice, FBI and Child Abuse Index Check.

At this time, the facility is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today.

LPA Rivera provided technical assistance to place a barrier between the kitchen/stairs and hallway to prevent access to children entering the area.

LPA provided The Notice of Site Visit (LIC 9213) and explained that it must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.



Upon receipt of this report, the Licensee shall post the Notice of Site Visit and any Licensing report documenting a type “A” deficiency.
Exit interview was conducted with Director. LPA explained and discussed this report, including the appeal rights and provided a copy. The signature on this report acknowledges and confirms receipt of these documents.
