On 9/24/2020 at 11:28 am Licensing Program Analyst (LPA) Dalicia Adkins conducted a tele-visit investigation with principal Carol Collins. The purpose of today's visit 9/24/20 is to deliver finding of complaint, director was informed of today's tele-visit. No chidlren were present during today's tele-visit, the center is currently closed (inactive-covid 19) due to covid-19 pandemic shut down.

On 5/1/20 LPA Adkins conducted tele-visit with principal Carol Collins and the following records were collected for review: (1) Staff Schedule (2) Teacher Roster with contact numbers, (3) Parent Contact Roster, (4) Activity Schedule
LPA Adkins interviewed Director on 5/1/20, 9/23/20 and 9/24/20. Parents were interviewed during the course of this investiagtion. There is insufficient evidence to prove that there was a violation of children personal rights.

In accordance with California Code of Regulations Title 22, HYDE PARK EARLY EDUCATION CENTER did not receive any deficiencies during the course of this investigation.

Based on record reviews and interviews it was determined that the above allegation to be unsubstantiated, meaning that the allegation evidence was insufficient to satisfy the preponderance of the evidence standard.

Exit interview conducted with Director Carol Collins. This report was sent via email with read receipt notification to verify that director received this report.
