25 | Licensing Program Analyst (LPA) Denise Gibbs conducted an unannounced case management inspection on 2/1/23 at 8:45am. Upon arrival, LPA met with Raquel Ramirez, Office Manager. Carolyn Macaranas , Principal arrived shortly after. There were 85 children present during inspection.
On 1/27/23, Principal submitted an unusual incident report to the Department regarding an incident that occurred that same day. Incident report stated that a Child One (C1) left the school premises, running down the sidewalk to the Hooper Primary Center entrance. LPA conducted interviews with staff and child involved. LPA also inspected all entrances and exits to the facility. Information gathered through interviews and observations confirmed that child was in eye sight of a staff member at all times. On this day C1 was exhibiting challenging behaviors, S1 was assisting in their classroom for extra support. Staff One (S1) and Staff Two (S2) were supervising C1 in the restroom. LPA observed three separate restrooms along one side of wall with cubbies along the opposite wall. There is a narrow walkway between the restroom doors and the cubbies, approximately 2ft wide. Per S1, C1 was in the walkway upset because they did not want to wash their hands. As S1 was assisting another child in the restroom, C1 ran around the corner to the right toward the main entrance door. S2 followed right behind C1. LPA observed the distance from the walkway to the main entrance door is approximately 20 steps. The door has a push bar that opens out. C1 pushed the bar to the main entrance door and ran out of the metal security gate that leads to the sidewalk. Security gate was open. Per Principal, security gate has not been working for approximately two weeks. During this time S2 was still supervising C1. S3 was in the parking lot on a break and immediately assisted in supervising C1 and returning them to the facility. C1 remained on the same side of the block, did not cross the street and was stopped at Hooper Primary Center to the right of the school. C1 was not injured and Principal met reporting requirement for this incident.
Per Principal, behavior support is scheduled for C1 in conjunction with parent. Additional staff is available to assist in the classroom when challenging behaviors occur. A staff meeting is scheduled for 2/2/23 to discuss --------------PAGE 1 |
