11/17/2021 Licensing Program Analyst (LPA) Lisa Clayton conducted a case management visit to follow up on an Unusual Incident Report, self reported by the facility on 11/03/2021. The El Segundo Child Care Regional Office received the report via fax on 11/09/2021. LPA was greeted by Principal Lewita Shatee. Also present were 20 staff members and 38 children in care. LPA toured the facility for a Health and Safety inspection and observed children in care being handled and supervised appropriately.

According to the incident report, on 11/03/2021, a parent called in at 3:05pm to say that her son C1 had a head injury that was made visible when she washed his hair. C1 is picked up between 12:00-12:30pm every day. Parent said he had scratches and bruises near his forehead and hairline. She didn't notice any injuries when she picked him up and that the child was happy, but she wanted to know where it came from. She wanted to know if he fell or if something happened. There were no reports of an injury to C1 that day. Because teachers leave at about 2:45PM, Principal has not had the chance to speak with the teachers. The behavioral interventionist was with the child all day on 11/03/2021. It was his first day at the school with C1. He came to observe C1's behavior.

On Friday 11/05/21 Principal Shatee spoke with the Behavioral Interventionist (BI) and he reported that C1 had thrown a dinosaur toy in the air and it came down and hit his head (he is known to throw things - C1 has Downs Syndrome) but he didn't cry or complain and BI didn't notice any bruising or scaring. C1 returned to school the next day and the staff only saw a tiny scratch and attempted to take a picture of it but C1 wouldn't sit still and allow the photo to be taken.
C1 wasn't available to interview today and has been absent since 11/08/2021 per mom's emails due to a runny nose.
LPA Clayton spoke to the child's teacher who reported that Mom does a health check of the child everyday and looks at his body before they leave the facility. When C1 returned the next day the child appeared fine and she only observed a tiny scratch on his forehead under his bangs which appeared to be something done with his own fingernails.
Based on the facts gathered and interviews conducted, the facility appeared to be in compliance during the incident on 11/03/2021. It does not appear the incident was the result of a Title 22 and/or Health & Safety Code violation. Although the staff was in close proximity to C1 when the incident occurred, they were unable to prevent the incident from occurring.

There are no deficiencies cited during today's inspection. An exit interview was conducted, a copy of this report and notice of site visit provided to Principal Shatee.
