25 | On 07/13/2023, Licensing Program Analyst (LPA) Lisa Clayton conducted an unannounced Case Management – Incident inspection related to a self-reported Unusual Incident Report (UIR) submitted on 07/07/2023. Upon arrival, LPA met with Principal Lewita Shatee, and explained the purpose of the visit. LPA observed 19 children in care, being supervised by 9 teachers/aides.
According to the UIR, on 07/07/2023, C1 was outside playing a game of Ring around the Rosy with teacher S1 and his classmates, and when they got to the “all fall down” he fell, and hurt his head. According to S1 after the incident, he put his hand on his head and when asked by S1, C1 said nothing was wrong, and according the her, an ice pack was given to him. When his father picked him up, he took the child to the car and brought him back, because he was crying and complaining about his hand. Dad was asking what happened to the child’s hand, and S1 was trying to talk to the child, who would not look at her, she bent down and attempted to turn his face to her, and Dad alleges that she slapped him. The father took the child to urgent care for his hand.
Dr. Shatee called the police department on 07/10/2023 and reported the incident.
During today’s inspection, LPA toured the indoor and outdoor of the facility, spoke with Principal Shatee, interviewed staff and C1 and P1. LPA received a copy of the children’s roster for review.
At this time, further investigation is needed.
An exit interview was conducted. A copy of this report (LIC 809) and Notice of Site Visit were provided to Principal Lewita Shatee, and must remain posted for 30 days.
