Licensing Program Analyst (LPA) Susann Sanchez conducted an unannounced complaint inspection. Upon arrival LPA met with Ana Vidal, Principal. Census was taken. LPA informed Principal a complaint has been received regarding an alleged violation above of the personal rights of a child. During the course of the investigation interviews were conducted with staff, children, and the director.

During interviews conducted with child #3. The child disclosed that staff #6 screams at the children that do not listen. Based on the interviews conducted during previous visits, the complaint allegation is valid because the preponderance of evidence standard has been met, therefore the allegation is found to be substantiated.

Exit interview conducted with the Principal. A copy of the appeal rights (LIC9058 01/16) were provided. The Principal signature on this report acknowledges receipt of her rights and a copy of this report was provided. Upon receipt, Principal posted the Notice of Site Visit. The Notice of Site Visit shall be posted for thirty (30) consecutive days. Failure to maintain posting as required will result in a $100.00 civil penalty.

Buildings and Grounds
The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement was not met as evidenced when Room #3 was closed due to a musty smell and moisture found in the storage wall on 08/27/2019.

Licensing Program Analyst (LPA) Susann Sanchez conducted an unannounced complaint inspection. Upon arrival LPA met with Ana Vidal, Principal. Census was taken. LPA informed Principal a complaint has been received regarding an alleged physical plant violation above. An investigation was conducted regarding the complaint allegations listed above. During the course of the investigation interviews were conducted with staff, children, and the director.
On 8/27/18 Room #3 was closed due to a musty smell and moisture found in the storage wall. Principal self reported allegation on 08/27/2018 and a follow- up visit was conducted by LPA Ramos on 10/18/2018. Based on the interviews conducted during previous visit and documentation gathered, the complaint allegation is valid because the preponderance of evidence standard has been met, therefore the allegation is found to be substantiated.

Based on interviews with children and staff, there were no disclosures made regarding the above allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.
