Licensing Program Analyst (LPA) Denise Gibbs conducted an unannounced case management inspection on 6/20/23. The purpose of this visit is to address incidents that were reported to the department. Upon arrival, LPA met with Christiana Park, Principal. There were 95 children and 20 staff present during inspection.

LPA is following up on incidents reports that were reported. All incidents were reported timely. LPA interviewed staff regarding each incident. Child (C) C1 did not require medical attention. Paramedics were called to the facility but were not needed because staff was able to dislodge child arm from apparatus. Child was back at school the next day. There was no injury to child's arm. LPA observed and assessed the safety of the apparatus.

Facility reported a water leak to the department. No children were injured during this incident. Children were safely relocated to Room #3 while water damage to Rooms #4 and #5 was repaired by the district. LPA observed and assessed the repair to the ceilings in Rooms #4/5. Ceiling tiles were observed to be repaired and safe. No water damage observed.

C2 did get medical attention for swollen eye that was observed when they woke up from nap. Per staff, mom took child to the doctor, who mentioned child may have been bit by an insect. Child was back at school the next day with no mark on eye. No other children were observed to have bug bites in the classroom.

Facility reported incident with C3 to all required parties. Incident did not occur at the facility.

On 6/12 Principal reported a possible violation of C4 personal rights. C4 alleged that Staff (S) S1 hurt their arm. LPA conducted interviews and observed video of incident. Further information is needed.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days.

Exit interview was conducted with Principal, Christiana Park, during which appeal rights were explained.
