Licensing Program Analyst (LPA), Keyona Scott, conducted an unannounced Case Management - Incident inspection at the Child Care Center on 02/26/2025, for the purpose of investigating an incident that occurred on 02/06/2025 of a child who eloped classroom and was found unsupervised in the children's restroom. LPA met with facility representative (S1) at 9:58 AM. LPA was guided on a tour inside and outside of the facility. LPA observed the following appropriate teacher to child ratios at 10:05 AM: thirteen (13) children to one (1) teacher and three (3) Aides in Room 1; seventeen (17) children to one (1) teacher and four (4) Aides for Room 2, who were outside playing; seventeen (17) children to two (2) teachers and three (3) Aides in Room 3; twelve (12) children to one (1) teacher and three (3) Aides for Room 4, who were outside playing; 13 children to one (1) teacher and two (2) Aides in Room 5. As a condition of employment, all individuals working or present at the facility that is operated by a school district shall obtain a criminal record clearance or criminal record exemption. The school district shall notify the Regional Office, if there is a change of status of an individuals criminal record clearance.

During today's inspection, LPA obtained the following documents: Classroom Rosters, Child Care Facility Roster (LIC 9040).

During today's inspection, LPA conducted interviews and facility observation.

During interviews, LPA observed child leave to use two different restroom locations without requesting assistance from staff.

During inspection, LPA observed classroom door to Room 1 open during operation hours. Staff confirmed that classroom door to Room 1 is normally left open during the day and only closed during drop off. LPA did not observe any preventative measure at the open classroom door to prevent children from exiting the classroom without supervision. PAGE 1

Based on observation and information obtained, the preponderance of evidence standard has been met, the facility did not ensure that there was supervision of all children in care at all times; therefore, the incident of LACK OF SUPERVISION, child eloped classroom and was found in children's restroom unsupervised, is SUBSTANTIATED. California Code of Regulation, Title 22, 101229(a)(1) is being cited on the attached LIC 809-D.

LPA, Keyona Scott, informed facility representative (S2) that this report dated 02/26/2025 documents one (1) Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA, Keyona Scott, informed the facility representative (S2) to provide a copy of this licensing report dated 02/26/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A notice of site visit was given and must remain posted for 30 days.
