On 10/24/2019 at 2:15 pm, Licensing Program Analyst (LPA) Denise Miranda and Lisa Rios conducted an unannounced inspection at Westminster Early Education Center for the purpose of following up on the self-reported incident that occurred at the facility on 06/252019.
Upon arrived Ms. Lanetta Grant, Office Manager, informed to LPA that Ricky Lim, Principal and Dr. Roper, Director were not present. There was a total census of 60 children in care with 18 staff.

According to the report, on 06/25/2019, Staff#2 and #3 observed child#1 outside of the classroom on his cot. Child#1 stated that staff#1 placed him outside of the classroom.

During this visit LPAs interviewed teacher#1 and #2.
At this time, further investigation is needed.
An exit interview was conducted and a copy of this report along with the Notice of Site Visit were provided to Lanetta Grant, Office Manager.
