25 | On 10/11/2019 at 2:15 Pm, Licensing Program Analyst (LPA) Denise Miranda conducted an unannounced case management incident inspection at Westminster Early Education Center for the purpose of following up on the self-reported incident that occurred at the facility on 06/252019. LPA informed the Ricky Lim, Principal the purpose of this visit..
There was a total census of 57 children in care with 12 staff and the Principal. During this inspection the Director was not present.
According to the report, on 06/25/2019, Staff#2 and #3 observed child#1 outside of the classroom on his cot. Child#1 stated that staff#1 placed him outside of the classroom.
During this inspection, LPA obtained copies of the following documents:

· The California Standards for the Teaching Profession

· Training conducted 9/20/2019 – Attendance, Communication, child abuse, children’s safety, lesson plan, outdoor expectation.
· Training conducted 9/24/29 – Safe School Plan – discuss the staff roles and responsibilities
· Training conducted 10/11/19 - Outdoor Expectation
· Training will conduct 10/17/2019 – Safety Environment.
* Principal will send no later than 10/14/2019 sign in/sign out staff and children in care dated on 6/25/2019. Copy and proof of the Unusual Incident Report that was submitted to El Segundo Regional Office, and declarations from the staff regarding to this incident..
At this time, further investigation is needed.
An exit interview was conducted and a copy of this report along with the Notice of Site Visit were provided to Ricky Lim, Principal. |
