Licensing Program Analysts (LPAs) Denise Gibbs and Rosaura Valenzuela conducted an unannounced annual required inspection at the above facility on 5/1/23 at 11:30 AM. LPA met with Rosa Preston, Facility Representative, who guided analysts on a tour of the facility.

This is a preschool program that operates Monday-Friday from 7:00AM - 5:30PM.

There were 33children and 13 staff present when LPA arrived. Facility is in compliance with license capacity and Title Five ratio guidelines. All individuals present have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Los Angeles Unified School District.

Facility has three classrooms. During inspection classrooms #1 and #2 were being used. LPAs observed that Room#3 (Gereral Ed.and PCC) is closed and made inaccessible. Per FR, the floors are being replaced in the classroom and classroom will be reopened on 5/4/23. LPA discussed unusual incident reporting with FR. All classrooms had furniture in good condition, free of loose, sharp and/or pointed parts. The floors and surfaces in the classrooms were clean and safe. Rugs were observed to be flat on the ground to avoid tripping hazards. Water is made readily available via operable water fountain in Room#1 and water bottles and water jug in Room #2. There are currently children with medication. LPAs observed that medication is stored in a locked cabinet located in the main office, inaccessible to children. Children have cubbies to store personal belongings separate from each other. LPAs observed children using cots to nap. All cots were observed to have required bedding. Bedding is provided by facility and washed weekly by laundry service. There is extra bedding on hand to change as needed. LPAs toured the children’s restrooms. Restrooms were observed to be safe and sanitary with operable sinks and toilets. Facility has conducted required water lead testing with no Action Level Exceedance (ALE) found. LPA discussed posting for lead results with FR.

LPAs toured outdoor play area. Due to the rain, the out door area was not set up for play. Outdoor area was ------------------(page 1 of 3)

observed to have age appropriate toys and material for children, free of loose, sharp, and/or pointed parts. LPAs observed required cushioning under climbing structures to absorb fall. Shade was observed throughout the outdoor area. Water is made readily available via operable water fountains and children take water bottles outdoors.

LPAs observed required posted documentation in the hallway, the teachers lounge, throughout the main office, and in the main entrance. The postings include Facility License, Publication (PUB) 393- Notification of Parent Rights, Licensing Form (LIC) 610- Facility Disaster Plan, PUB 269- Child Passenger Restraint System, LIC 613A- Notification of Personal Rights and Lunch/Snack Menu. Facility Disaster plan was discussed.

Facility records were reviewed for LIC 9040- Facility Roster, Daily schedules and Disaster drill log; last drill conducted on 4/11/23. All items were observed.

LPAs toured the kitchen on site. Facility provides Breakfast, Lunch and Snack. Food is not cooked at the facility. Prepackaged food is brought from the neighboring, elementary school. Extra food is taken back to elementary. Kitchen was observed to be clean, free of litter, insects and rodents. LPAs observed a trash can for solid waste to have a tight fitting lid. Cleaning supplies are stored separate from the food, in the hopper room and in a cabinet under the sink. LPAs informed that if the kitchen door remains open while children are in care, the cabinet under the sink will need to be locked. Carbon monxide detector is wired with fire and smoke alarm.

LPAs reviewed Sign In/Out sheets located at the drop off and pick up table. All children present were signed in with date, time and full signature of the child's guardian.

Children’s records were reviewed for Emergency Card, Immunization Records, Licensing Form (LIC) 627- Consent for Medical Treatment, LIC 995 Notification of Parents’ Rights, LIC 701- Physician’s Report, LIC 613A- Personal Rights, Signed request for medication form, medication prescription and expiration date, and signed Admissions Agreement. LPAs observed C1 medication did not have a prescription label, instructions for medication was handwritten on a paper inside the bag with the medication. C3 is not in care today.

Staff records were reviewed for approved Pediatric First Aid and CPR certification, LIC 9052- Employee Rights, Proof of immunization against measles, pertussis and influenza or influenza declination, TB clearance -------------------------(page 2 of 3)

or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse, Transcripts or Permit and current Mandated Reporter Training Certificate. Teachers permits were missing for 5 out 5 staff.

During inspection all children were observed to be treated with dignity and respect, they were observed to be receiving safe, healthful and comfortable accommodations, furnishings and equipment, and free from corporal and/or unusual punishment.

Incidental Medical Services (IMS):
This facility provides Incidental Medical Services- IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personal, and administrative records. Based on the LPA’s observations and records review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.
