On 07/11/2024 at 10:45 AM an Unannounced Annual/Random Inspection was conducted by Licensing Program Analysts (LPAs) Katrina Chicote and Peter Bishop. LPAs announced purpose of visit and met with Facility Representative (FR), Curtis Johnson, Principal. Facility is currently licensed for a capacity of 115 and per Facility Roster have 62 enrolled. The program currently operates Monday thru Friday from 7:00 AM to 5:30 PM. LPAs were taken on a guided tour of the facility of both indoors and outdoors. Required postings were observed. LIC 125 was provided.

The facility is made up of four classrooms: RM-1, RM-2A/2B, and RM-3. At the initial start of the site visit, LPAs observed RM-1 to have 11 children with four staff playing outside, RM-2A to have nine children with three staff, RM-2B to have eight children with three staff playing outside, and RM-3 to have eight children with three staff. Total census of 36 children and a total census of 14 staff present during today's inspection.

Beginning at 11:00 AM, furniture and equipment were inspected for age appropriateness and good repair in each classroom. LPAs observed surface of classrooms to be linoleum and rugs which were maintained and safe and clean at time of inspection. LPAs observed child size tables and chairs available for children with rounded edges and well maintained. LPAs observed learning stations for dramatic play, math, and science with a variety of age appropriate toys maintained free of loose and sharp parts and a library area with soft seating available for quiet play. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings in hallway. Cots were inspected and observed stored in closets in classroom. Per FR linens are provided by the facility and washed by weekly laundry service. Availability of drinking water was reviewed. LPAs observed drinking fountains available in each classroom for children as well as children's personal water bottles. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. sinks available, LPAs observed RM-3 to have own bathroom that had 1.5 toilets and one sink. Hazardous items including poisonous cleaning compounds were stored inaccessible to children. Per FR, only LAUSD approved disinfectants are present at the facility which are stored outside of classrooms.

A first aid kit is kept in a backpack in each classroom and was inventoried for supplies. At 11:00 AM, LPAs tested carbon monoxide detector, located in hallway by Kitchen, and observed to be operable at time of inspection. LPAs observed smoke detectors and fire extinguishers are present in the facility. LPAs discussed requirement to have fire extinguishers serviced annually and to conduct regular checks of smoke and carbon monoxide detectors to ensure they are in operable condition. Disaster drill log was available and last drill conducted on 06/25/2024. Isolation area for sick children is in Teacher's Lounge where LPAs observed child seating available and will have full supervision until child is picked up by Authorized Representative. Per FR, they currently have two children (C1 and C2) that require medicines. LPAs observed medicines stored in locked closet in Teacher's Lounge which is centrally located but inaccessible to children. LPAs observed medicines for children in original packaging with unaltered label and conducted record review to ensure completion of required documents.

Snack/lunch menus were reviewed and observed posted. Facility provides Breakfast, Lunch, and PM Snack. Per FR, facility does not cook food on site and is delivered from nearby elementary school campus. Food preparation areas were toured for safety, cleanliness and proper equipment. Trash cans used to discard food have tight fitting lids.

Beginning at 11:20 AM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPAs observed two outdoor play areas available: Outdoor Play Area 1 and Outdoor Play Area 2. LPAs observed surface of outdoor play areas to be safe and maintained for activities planned. LPAs observed a variety of age appropriate toys available used for music and movement, sensory areas, dramatic play houses, and large manipulatives made for outdoor play that were well maintained at time of inspection. Required shade and fencing were inspected. LPAs observed two age appropriate structures for children that were well maintained with cushioning material below to absorb fall. LPAs observed vegetable gardens in outdoor play areas that are maintained. LPAs advised FR that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to drinking fountains and their personal water bottles during outside play. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. LPAs observed all children present signed in at time of inspection. Personal Rights of children were observed by LPA.

Beginning at 12:00 PM, staff and children’s records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857 for children and the LIC 859 for staff. LPAs did not review confidential staff files during the inspection due to the staff confidential files being stored at the administrative office. CPR card expires on 09/2025 for Staff #1. 4 of 6 staff files reviewed had proof of completion of AB1207 Mandated Reporter training on file at time of inspection. All staff and employees have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Los Angeles Unified School District (LAUSD).

The following were discussed:
LPA advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation, this includes any changes as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location.

LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

CCC COMPLETED TESTING AND HAS REMEDIATED ALEs:
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited but advisories given today, 07/11/2024.

A notice of site visit was given and must remain posted for 30 days.
