Licensing Program Analysts (LPAs) Raul Navarro and Patricia Medel arrived at the above licensed facility to conduct an unannounced Case Management inspection on 01/26/2023. LPAs arrived at the facility at 9:20am and met with Site Supervisor Denise Calderon, who guided LPAs on a tour of the facility. There were 43 children present and 11 staff during today's inspection.

The Monterey Park South West Child Care Regional Office received the incident report on 12/13/2023 regarding a child sustaining an injury that required medical attention. LPA's interviewed Site Supervisor Denise Calderon, Associate Teacher, Maria Holguin, Lead Teacher, Lace Miranda, and Pre-School Teacher, Jennifer Rodriguez. Information obtained through interviews were consistent with the information provided in the incident report. LPA's toured the play area where child fell. LPAs did not observe any objects in the play area that could be hazardous to children in care. Site Supervisor provided LPAs with a video that shows the incident. In the video it is seen that there was enough staff supervising the children in care. Child bumped into another child causing them to fall. Video shows that the incident was an accident and could not be prevented. Parent was notified in a timely manner.

According to the available information, it does not appear this incident was the result of the Title 22 violation for lack of care and supervision.

Exit interview conducted and report was reviewed with Site Supervisor Denise Calderon.

No deficiency was cited at this time. A notice of site visit was given and must remain posted for 30 days.
